摘 要:目的分析肉芽肿性乳腺炎(GM)与非特殊型浸润性乳腺癌(IBC-NST)的超声特征差异,及其与病理表现的相关性,为两者的鉴别提供依据。方法选取经病理证实的161例(164个病灶)GM和294例(295个病灶)IBC-NST患者的临床、超声及病理资料,分析其超声特征与病理表现的相关性。结果GM和IBC-NST在病灶回声模式(低回声、囊实混合回声)、纵横比、边缘、高回声晕、延伸方式、后方回声、周边导管、钙化、血流方面比较差异有统计学意义(P<0.01),在病灶形态、病灶回声模式(无回声)方面比较差异无统计学意义(P>0.01)。GM及IBC-NST两组部分超声特征及病理表现相关性显示,二者基本一致,多数比较差异无统计学意义(P>0.01)。结论GM与IBC-NST的超声特征差异有助于二者的鉴别诊断,且二者的超声特征与病理表现多有相关性。Objective To analyze the differences in ultrasound features between granulomatous mastitis(GM)and invasive breast carcinoma of no special type(IBC-NST),and their correlation with pathological findings,providing a basis for their differentiation.Methods The clinical,ultrasound and pathological data of 161 cases(164 lesions)of GM and 294 cases(295 lesions)of IBC-NST confirmed by pathology were selected to analyze the correlation between their ultrasound characteristics and pathological manifestations.Results There were statistically significant differences between GM and IBC-NST in terms of lesion echo pattern(hypoechoic,cyst-solid mixed echo),aspect ratio,edge,hyperechoic halo,extension pattern,posterior echo,peripheral duct,calcification,and blood flow(P<O.01),there was no statistically significant difference in lesion morphology and lesion echo pattern(no echo)(P>O.O01).The correlation of some ultrasound features and pathological manifestations between the two groups of GM and IBC-NST showed that they were basically consistent,and most of the differences were not statistically significant(P>0.O1).Conclusions The differences in ultrasound features between GM and IBC-NST are helpful for their differential diagnosis,and the ultrasound features of both are often correlated with pathological manifestations.
