缩窄性心包炎患者的回顾性分析及其超声诊断标准的建议

摘　　要：目的总结分析30例缩窄性心包炎(CP)患者的超声心动图和临床特征,探讨CP的超声诊断标准。方法选择30例CP患者,总结临床表现和超声心动图的二维、频谱及组织多普勒等特征。结果CP患者主要临床表现为乏力、胸闷和劳力性呼吸困难、下肢水肿、肝淤血和颈静脉充盈。超声特征包括心包增厚及钙化、心房扩大、心室限制性充盈、左心室后壁舒张期运动平直、室间隔震颤和弹跳、室间隔漂移、二尖瓣和三尖瓣血流呼吸变异率升高、二尖瓣环翻转、下腔静脉(IVC)扩张等。结论超声心动图是一种有效的诊断CP的检查方式,本研究提出了CP的超声诊断标准,填补了临床空白,且简单实用,值得推广。Objective To analyze the ultrasonic features of 30 patients with constrictive pericarditis(CP),and to propose the suggestions as to the echocardiographic diagnostic criteria.Methods Thirty patients with CP were selected for the study.The clinical and echocardiographic data were analyzed and summarized.Suggestions about diagnostic criteria of CP were put forward.Results The main clinical manifestations were fatigue,breathlessness and exertional dyspnea and signs of edema of lower limbs,hepatomegaly and jugular vein filling.Echocardiographic features included pericardial thickening and calcification,inferior vena cava(IVC)dilation,atrial enlargement,diastolic flattening of the LV posterior wall,septal shudder and bounce,restrictive LV and RV diastolic filling pattern,early filling changes of mitral and tricuspid flow.Conclusions Echocardiography is a preferred examination for CP.The echocardiographic diagnostic criteria are valid and might be further promoted.
