超声多普勒技术和压力波法测量肱-踝脉搏波传播速度一致性研究

摘  要:目的 探究超声多普勒技术与压力波法测量肱-踝脉搏波传播速度(baPWV)的一致性。方法 纳入确诊住院的冠心病患者30例,同期招募健康者30例,共60例为研究对象。由两名观察者分别采用一种基于超声多普勒法的动脉僵硬度自动测量新技术和压力波法测量研究对象的baPWV,对比分析两种方法测值间的一致性。结果 在60例研究对象中,采用压力波法测量的baPWV为(13.11±1.70)m/s,采用超声多普勒技术测量的baPWV为(12.76±1.69)m/s,两种方法测值对比,其差异无统计学意义(P>0.05),且两者间的组内相关系数为0.962(95%置信区间:0.932~0.978)。Bland-Altman分析显示两种方法具有高度的一致性[差值均值:(-0.30±0.85)m/s;一致性界限:-1.98~1.37 m/s]。此外,采用超声多普勒技术测量baPWV的观察者内及观察者间一致性分析显示,超声多普勒技术具有较高的可重复性。结论 在无创测量baPWV方面,超声多普勒技术与压力波法相比,具有极高的一致性和可重复性,有望为临床无创评估动脉僵硬度提供另一种方法。Objective To investigate the consistency between Doppler ultrasound technology and pressure transducers in measuring brachial-ankle pulse wave velocity(baPWV).Methods Thirty patients with coronary heart disease who were hospitalized as coronary heart disease group,and 3o healthy participants were recruited as hypertensive groups at the same time.baPwV was measured in all subjects by two independent observers using a new automatic measurement technology of arterial stiffness based on Doppler ultrasound and the pressure transducers.The measurements were compared and the agreement was analyzed.Results Among the 60 subjects,the baPWV measured by the pressure transducers was(13.11±1.70)m/s,and the baPWV measured by the Doppler ultrasound technology was(12.76±1.69)m/s,and there was no significant difference between the two methods,and the intraclass correlation coefficient was 0.962(95%CI:0.932-0.978).Bland-Altman analysis showed good agreement between the two methods[mean difference:(-0.30±0.85)m/s;limit of agreemen:-1.98-1.37 m/s].In addition,intra-bserver and inter-observer consistency analyse of the Doppler ultrasound technology for measuring baPWV showed that Doppler ultrasound technology had high reproducibility.ConclusionsCompared with the pressure transducers,Doppler ultrasound technology has high consistency and reproducibility in the non-invasive measurement of baPwV,which is expected to provide another method option for non-invasive clinical evaluation of arterial stiffness.
