经直肠超声造影联合MRI在PSA灰区前列腺癌诊断中的价值

摘  要:目的探讨经直肠超声造影(CEUS)、磁共振成像(MRI)及两者联合在诊断前列腺特异度抗原(PSA)灰区前列腺癌的应用价值。方法本研究纳入127例血清PSA灰区可疑前列腺癌的患者,均完成CEUS及MRI检查。所有患者行前列腺穿刺活检,以活检病理结果为“金标准”,对不同影像学方法组间诊断效能进行分析比较。结果本研究检出前列腺癌患者42例,其中CEUS检出31例,MRI检出34例,两者联合诊断检出39例;CEUS、MRI、联合诊断对PSA灰区前列腺癌诊断的受试者工作特征(ROC)曲线下面积分别为0.740、0.746、0.835;联合诊断的ROC曲线下面积大于单独应用MRI或CEUS,差异有统计学意义(P<0.05),而CEUS检查与MRI检查相比较两组间差异无统计学意义(P>0.05)。结论单独应用CEUS及MRI对PSA灰区前列腺癌的诊断能力相仿,CEUS联合MRI诊断能有效提高PSA灰区前列腺癌的检出率,提升前列腺癌早期诊断能力。Objective To explore the application value of contrast-enhanced ultrasonography(CEUS),magnetic resonance imaging(MRI),and their combination in the diagnosis of prostate-specific antigen(PSA)gray zone prostate cancer.Methods This study included 127 patients suspected of prostate cancer with serum total PSA levels in gray zone,all of whom underwent CEUS and MRI examinations.All patients underwent prostate biopsy,and the diagnostic efficiency analysis and comparison among different imaging methods were performed according to the pathology results as the"gold standard".Results In this study,42 cases of prostate cancer were detected,with 31 cases detected by CEUS,34 cases detected by MRI,and 39 cases detected by the combination of both.The areas under receiver operating characteristic curves of CEUS,MRI,and combination in diagnosis of PSA gray zone prostate cancer were 0.740,0.746,and 0.835,respectively.The area under ROC curve for combination was greater than MRI or CEUS alone,with statistical significance(P<O.05),while the differences between CEUS and MRI examinations showed no statistical significance(P>0.05).Conclusions The diagnostic capabilities of CEUS and MRI alone for PSA gray zone prostate cancer are similar.The combination of CEUS and MRI can effectively increase the detection rate of PSA gray zone prostate cancer and enhance the early diagnosis capabilities of prostate cancer.
