非产褥期子宫内翻的超声表现并文献回顾

摘　　要：目的探讨非产褥期子宫内翻的超声特点,为临床诊断提供依据。方法回顾分析总结1例非产褥期子宫内翻病例及文献检索获得的有详细超声表现的13例病例的超声图像特征。结果非产褥期子宫内翻的病因中恶性肿瘤占42.86%(6/14例),良性肿瘤占57.14%(8/14例),其中黏膜下子宫肌瘤最多见,占42.86%(6/14例)。非产褥期子宫内翻具有典型的超声图像特征:宫底内陷,宫腔呈“Y”或“U”字型的倒置,子宫中央可见多发线样高回声,纵切呈“套筒”征,横切呈“靶环”征;包块常与宫底相连,可位于宫腔下段、宫颈或阴道;双卵巢相贴呈“接吻”状;双侧子宫动静脉呈两束条状血流分别从子宫凹陷进入子宫中央。结论超声可作为非产褥期子宫内翻的首选检查,通过识别超声图像的典型特征可做出提示性诊断,帮助临床及时干预和治疗。Objective To explore the ultrasonic characteristics of non-puerperal uterine inversion and provide a basis for clinical diagnosis.Methods The ultrasound image characteristics of 1 case of non-puerperal uterine inversion and 13 cases obtained from literature with detailed ultrasound manifestations were retrospectively analyzed.Results The causes of non-puerperal uterine inversion were malignant tumors in 42.86%(6/14 cases)and benign tumors in 57.14%(8/14 cases),and submucosal uterine fibroids were the most common among which,accounting for 42.86%(6/14 cases).Non-puerperal uterine inversion had typical ultrasonic image features:fundal inversion,"y"or"U"inversion of the uterine cavity,multiple linear hyperechoic signs in the center of the uterus,"sleeve"sign showed in longitudinal insection,and"target ring"sign showed in transverse section.The mass often attached to the fundus and might locate in the lower uterine cavity,cervix,or vagina.The ovaries attach each other in a"kissing"shape.The bilateral uterine arteries and veins flowed as two strips from the uterine depression into the center of the uterus.Conclusions Ultrasound can be used as the first choice for non-puerperal uterine inversion examination,and can make a suggestive diagnosis by identifying the typical features of ultrasound images,and may helpful for clinical intervention and treatment in a timely manner.
