定神汤联合吴茱萸粉外敷治疗乳腺癌术后化疗期间失眠临床研究

摘　　要：目的:观察定神汤联合吴茱萸粉外敷治疗乳腺癌术后化疗期间失眠肝郁脾虚证的临床疗效。方法:选取80例乳腺癌术后化疗期间失眠肝郁脾虚证患者,根据随机数字表法分为对照组和治疗组各40例。对照组予以艾司唑仑片治疗,治疗组在对照组基础上加用定神汤口服、吴茱萸粉外敷治疗,2组均治疗14 d。比较2组临床疗效、睡眠质量、中医证候积分及不良反应发生率。结果:治疗14 d后,治疗组总有效率77.50%,高于对照组52.50%,差异有统计学意义(P<0.05)。2组匹兹堡睡眠质量指数(PSQI)评分均较治疗前降低,治疗组PSQI评分低于对照组,差异均有统计学意义(P<0.05)。2组中医证候积分均较治疗前降低,治疗组中医证候积分低于对照组,差异均有统计学意义(P<0.05)。治疗过程中,治疗组不良反应发生率17.50%,低于对照组37.50%,差异有统计学意义(P<0.05)。结论:定神汤联合吴茱萸粉外敷治疗乳腺癌术后化疗期间失眠可提高临床疗效,有效缓解患者的失眠症状,改善睡眠质量,减少不良反应。Objective:To observe the clinical effect of Dingshen Decoction combined with external application with Euodiae Fructus Powder on insomnia with the liver constraint and spleen deficiency syndrome during chemotherapy after surgery for breast cancer.Methods:A total of 80 cases of patients with insomnia with the liver constraint and spleen deficiency syndrome during chemotherapy after surgery for breast cancer were selected and divided into the control group and the treatment group according to the random number table method,with 40 cases in each group.The control group was treated with Estazolam Tablets,and the treatment group was additionally treated with the oral administration of Dingshen Decoction and the external application with Euodiae Fructus Powder based on the treatment of the control group.Both groups were treated for 14 days.Clinical effects,sleep quality,traditional Chinese medicine(TCM)syndrome scores,and the incidence of adverse reactions were compared between the two groups.Results:After 14 days of treatment,the total effective rate was 77.50%in the treatment group,higher than that of 52.50%in the control group,the difference being significant(P<0.05).The scores of the Pittsburgh Sleep Quality Index(PSQI)in the two groups were decreased when compared with those before treatment,and the PsQl scores in the observation group were lower than those in the control group,differences being significant(P<0.05).The TCM syndrome scores in the two groups were decreased when compared with those before treatment,and the TCM syndrome scores in the observation group were lower than those in the control group,differences being significant(P<0.05).During treatment,the incidence of adverse reactions was 17.50%in the control group,lower than that of 37.50%in the control group,the difference being significant(P<0.05).Conclusion:Dingshen Decoction combined with external application with Euodiae Fructus Powder can improve clinical effects on the treatment of insomnia during chemotherapy after surgery for breast cancer,effe
