阳和汤合附子汤联合牵引治疗寒湿痹阻型腰椎间盘突出症的临床疗效

摘　　要：目的:探讨阳和汤合附子汤联合牵引治疗寒湿痹阻型腰椎间盘突出症的临床效果。方法:将94例寒湿痹阻型腰椎间盘突出症患者按随机数字表法分为对照组和观察组各47例。对照组予间歇牵引治疗,观察组在对照组的基础上予阳和汤合附子汤治疗,2周为1个疗程,连续治疗2个疗程。观察两组治疗前后血清肿瘤坏死因子α(tumor necrosis factor-α,TNF-α)、白细胞介素1β(Interleukin-1β,IL-1β)表达水平及全血黏度、血浆黏度、红细胞聚集指数、视觉模拟量表(visual analogue scale,VAS)评分和Oswestry功能障碍指数问卷表(Oswestry disability index,ODI)评分,统计两组临床有效率及不良反应。结果:治疗后两组血清TNF-α、IL-1β表达水平及全血黏度、血浆黏度、红细胞聚集指数和VAS、ODI评分均较治疗前降低(P<0.05),且观察组上述指标水平低于对照组(P<0.05);观察组临床有效率为97.87%(46/47),高于对照组[82.98%(39/47)](P<0.05);两组均无明显不良反应发生。结论:阳和汤合附子汤联合牵引治疗寒湿痹阻型腰椎间盘突出症可提高临床疗效,减轻临床症状,促进腰部功能恢复,其作用机制可能与降低炎症因子水平,减轻局部炎性反应,改善血液黏滞度有关。Objective:To explore clinical effects of Yanghe Tang and Fuzi Tang combined with traction in the treatment of lumbar disc herniation(LDH)of cold-dampness obstruction pattern.Methods:All 94 patients were allocated to the control group and the observation group in light of random number table method,47 cases in each group.The control group was treated with intermittent traction,and the observation group orally took Yanghe Tang and Fuzi Tang based on intermittent traction,two weeks were one course of the treatment,and both groups were treated for two courses of the treatment continuously.To observe the expressions of TNF-αand IL-1β,whole blood viscosity,plasma viscosity,erythrocyte aggregation index,VAS scores and ODI scores before and after the treatment in the two groups,to compare clinical effective rate and adverse reaction between both groups.Results:After the treatment,the levels of TNF-αand IL-1β,whole blood viscosity,plasma viscosity,erythrocyte aggregation index,VAS scores and ODI scores were lowered than before the treatment in the two groups(P<0.05),and the above indexes of the observation group were lower than these of the control group(P<0.05);Clinical effective rate of the observation group was 97.87%(46/47),higher than 82.98%(39/47)of the control group(P<0.05);no obvious adverse reaction occurred in the two groups.Conclusion:Yanghe Tang and Fuzi Tang combined with traction in the treatment of LDH of cold-dampness obstruction pattern could improve clinical effects,relieve clinical symptoms,promote the recovery of lumbar function,and its mechanism might be related to lowering the levels of inflammatory factors,relieving local inflammatory response and improving blood viscosity.
