摘  要:目的:观察输尿管导管作为回水通路在输尿管镜下钬激光碎石术中的应用效果。方法:选取淄博市临淄区齐都医院2020年1月—2023年12月收治的输尿管结石患者182例,将其随机分为观察组和对照组,每组91例,均行输尿管镜下钬激光碎石术,观察组术中应用输尿管导管作为回水通路,对照组不使用输尿管导管作为回水通路。比较两组手术时间、碎石成功率、结石排净率、术后发热发生率及住院时间。结果:观察组的平均手术时间为(31.5±9.7)min,短于对照组的(37.1±12.1)min,差异具有统计学意义(P<0.05)。观察组术后发热的发生率为13.2%,低于对照组的27.5%,差异具有统计学意义(P<0.05)。两组患者碎石成功率、结石排净率、平均住院时间比较,差异无统计学意义(P>0.05)。结论:在进行输尿管镜钬激光碎石术时,置入一根F3~F5的输尿管导管作为回水通路,可以有效解决传统手术中存在的冲洗液回流不畅、视野模糊和肾盂压力升高的问题,能够缩短手术时间,降低术后发热发生风险。Objective To explore the application effect of ureteral catheter as a reflux pathway in ureteroscopic holmium laser lithotripsy.Methods A total of 182 patients with ureteral calculi admitted to Qidu Hospital,Linzi District from January 2020 to December 2023 were randomly divided into an observation group and a control group,with 91 patients in each group.Both groups underwent ureteroscopic holmium laser lithotripsy,with the observation group using ureteral catheter as a reflux pathway during the procedure,while the control group did not use ureteral catheter as a reflux pathway.The surgical time,stone fragmentation success rate,stone clearance rate,incidence of postoperative fever,and length of hospital stay were compared between the two groups.Results The average surgical time in the observation group was(31.5±9.7)min,which was shorter than that in the control group[(37.1±12.1)min],with a statistically significant difference(P<0.05).The incidence of postoperative fever in the observation group was 13.2%,lower than 27.5%in the control group,with a statistically significant difference(P<0.05).There was no statistically significant difference in stone fragmentation success rate,stone clearance rate and average length of hospital stay between the two groups(P>0.05).Conclusions Inserting a ureteral catheter of F3-F5 as a reflux pathway during ureteroscopic holmium laser lithotripsy can effectively address the issues of poor irrigation fluid reflux,blurry vision,and increased renal pelvis pressure in traditional surgery.This approach can shorten the surgical time and reduce the risk of postoperative fever.
