摘　　要：目的探讨一期经皮肾镜联合软镜治疗Guy’sⅢ~Ⅳ级肾结石患者的临床疗效。方法选取2021年8月至2022年2月赣县区人民医院泌尿外科收治的60例Guy’sⅢ~Ⅳ级肾结石患者作为研究对象,按照随机数字表法分为对照组与观察组,各30例。对照组采用经皮肾镜治疗,观察组采用一期经皮肾镜联合软镜治疗,比较两组手术指标、生化指标及并发症发生情况。结果两组术中出血量比较差异无统计学意义;观察组手术时间、住院时间均短于对照组,住院费用少于对照组,术后残留结石率和二次手术取石术率均低于对照组,差异有统计学意义(P<0.05)。术后7d,观察组血红蛋白水平高于对照组,血白细胞、尿白细胞与血肌酐水平均低于对照组,差异有统计学意义(P<0.05)。两组并发症发生率比较差异无统计学意义。结论一期经皮肾镜联合软镜治疗Guy’sⅢ~Ⅳ级肾结石患者疗效显著,能缩短患者住院时间,减少住院费用,降低二次手术取石率,改善患者肾功能,且不增加并发症发生率,值得临床推广应用。Objective To investigate the clinical effect of one-stage percutaneous nephroscopy combined with soft lens in the treatment of Guy's gradeⅢ-Ⅳkidney stones.Methods 60 patients with Guy'sⅢ-Ⅳkidney stones admitted to the Department of Urology in Ganxian District Peo-ple's Hospital from August 2021 to February 2022 were selected as the study subjects,and they were divided into the control group and the observa-tion group according to random number table method,with 30 cases in each group.The control group was treated with percutaneous nephroscopy,and the observation group was treated with one-stage percutaneous nephroscopy combined with soft lens,the surgical indexes,biochemical indexes and complications were compared between the two groups.Results There was no statistically significant difference in intraoperative bleeding be-tween the two groups;the surgery time and hospitalization time in the observation group were shorter than those in the control group,and the hospi-talization cost was lower than that in the control group,and the differences were statistically significant(P<0.05).The postoperative residual stone rate and secondary surgical stone removal rate were lower than those in the control group,and the differences were statistically significant(P<0.05).After 7 d of surgery,the hemoglobin level in the observation group was higher than that in the control group,and the levels of blood white blood cells,urine white blood cells,and blood creatinine were lower than those in the control group,and the differences were statistically significant(P<0.05).There was no statistically significant difference in the incidence of complications between the two groups.Conclusion One-stage percutane-ous nephroscopy combined with soft lens is effective in the treatment of Guy's gradeⅢ-Ⅳkidney stones,which can shorten the length of hospital stay,reduce the hospitalization cost and the rate of second operation stone removal,improve the kidney function of patients,and does not increase the incidence of complications,w
