利培酮联合阿立哌唑对男性精神分裂症患者内分泌激素水平的影响

摘　　要：目的探讨利培酮与阿立哌唑联用对男性精神分裂症患者内分泌激素水平的影响。方法将107例男性精神分裂症患者根据用药方案分为对照组(54例,单一利培酮)和观察组(53例,口服利培酮联合阿立哌唑),治疗前及治疗8周后分别检测两组患者血清中游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)、三碘甲状腺原氨酸(T3)、甲状腺素(T4)、促甲状腺激素(TSH)和泌乳素(PRL)浓度,计算不良反应发生率,并进行相关统计学分析。结果治疗8周后,对照组TSH水平较治疗前明显升高(P<0.05),且较观察组治疗后TSH水平明显升高(P<0.05),但观察组TSH水平与治疗前无明显差异,两组FT3、FT4、T3、T4均较治疗前无明显差异,且组间亦无明显差异;两组PRL水平均较治疗前明显升高(P<0.05),对照组血清PRL升高水平显著高于观察组(P<0.05)。两组不良反应发生率无明显差异。结论在利培酮治疗基础上联用阿立哌唑有助于抑制利培酮引起的PRL升高,可能减低其对甲状腺功能的影响,且未观察到不良反应的增加,有重要的临床联用价值。Objective To determine the effect of combination of risperidone and aripiprazole on the endocrine hormone levels in male patients with schizophrenia.Methods Totally 107 male patients with schizophrenia were divided into a control group(54 patients,taking oral risperidone)and an observation group(53 patients,taking oral risperidone and aripiprazole)according to the treatment plan.Before the treatment and 8 weeks after the treatment,the serum levels of free triiodothyronine(FT3),free thyroxine(FT4),triiodothyronine(T3),thyroxine(T4),thyroid stimulating hormone(TSH)and prolactin(PRL)in both groups were measured.The incidence of adverse reactions was calculated,and statistical analysis conducted.Results After 8 weeks of treatment,the level of TSH in the control group was higher than that before the treatment(P<0.05),and also higher than that in the observation group after the treatment(P<0.05).No significant difference was found before and after the treatment in the observation group.There were no obvious difference in the levels of FT3,FT4,T3,and T4 in both groups before the treatment,and no significant difference between the groups.The level of PRL in both groups was higher than that before the treatment(P<0.05),but the level in the control group was much higher than that in the observation group(P<0.05).There was no obvious difference in the incidence of adverse reactions between both groups.Conclusion Combination of aripiprazole and risperidone can help suppress the elevation of PRL caused by risperidone and may reduce its impact on the thyroid function,without increasing adverse reactions.
