浅析湖湘当代名中医治疗痹证的用药特点

摘　　要：目的:基于数据挖掘技术探寻湖湘当代名中医治疗痹证的处方用药特点。方法:选取《湖湘当代名医医案精华》丛书和《国医大师熊继柏临床现场教学录》中的痹证医案,运用中医传承辅助平台V2.5软件建立痹证处方数据库,统计中药频次、中药功效分类、证型分布、药物归经、四气五味、关联药组及复杂网络分析。结果:共纳入处方316首,中药210味,常用的高频药物有甘草、当归、黄芪、牛膝、白芍、苍术、川芎、秦艽、鸡血藤、薏苡仁等,功效分类排名前五的中药为补虚药、祛风湿药、活血化瘀药、解表药和清热药,排名前五的证型为风寒湿痹证、湿热瘀阻证、湿热阻痹证、痰瘀互结证与肝肾亏虚证。四气以温性为主,五味以苦味为主,归经主要涉及肝经、脾经、心经、肾经,关联规则分析排名前三的药物组合为川芎、黄芪、当归,黄柏、薏苡仁、苍术,黄柏、秦艽、苍术,聚类分析得到10个核心药物组合、5个新方组合。结论:湖湘当代名中医治疗痹证时以祛风湿、扶正补虚、活血化瘀法为主,同时注重调和营卫与清热化湿。Objective:Based on data mining technology to explore the characteristics of the Bi syndrome(痹证).Methods:The OBi syndrome cases in the series of Huxiang Contemporary Famous Medical Records Essence and XIONG Jibai Clinical Field Teaching were selected,and the arthralgia prescription database was established by the Traditional Chinese Medicine Inheritance Support System V2.5 software.The frequency,the classification efficacy,the distribution of syndrome types,the meridian tropism of drugs,the four Qi(气)and five flavors of TCM medicine,the related drug group and the complex network analysis were performed.Results:A total of 316 prescriptions and 210 TCM medicines were included.The commonly used high-frequency drugs were Gancao(Radix Glycyrrhizae),Danggui(Radix Angelicae Sinensis),Huangqi(Radix Astragali),Niuxi(Radix Achyranthis Bidentatae),Baishao(Radix Paeoniae Alba),Cangzhu(Rhizoma Atractylodis),Chuanxiong(Rhizoma Chuanxiong),Qinjiao(Radix Gentianae Macrophyllae),Jixueteng(Caulis Spatholobi),Yiyiren(Semen Coicis),etc..The top five TCM medicine efficacy classifications were tonifying drugs,wind-dampness-dispelling drugs,blood-activating and stasis-dissolving drugs,exterior-relieving drugs and heat-clearing drugs.The top five syndromes in syndrome analysis were wind-cold-dampness arthralgia syndrome,damp-heat stasis syndrome,damp-heat arthralgia syndrome,phlegm-blood stasis syndrome and liver-kidney deficiency syndrome.The four Qi was mainly warm,and the five flavor was mainly bitter.The meridian tropism mainly involved liver,spleen,heart and kidney meridians.The top three drug combinations in association rule analysis were Chuanxiong-Huangqi-Danggui,Huangbai(Cortex Phellodendri Chinsis).Yiyiren-Cangzhu,and Huangbai-Qinjiao-Cangzhu.Cluster analysis obtained 10 core drug combinations and 5 new prescription combinations.Conclusion:In the treatment of the Bi syndrome,contemporary famous TCM practitioners in Huxiang area mainly focus on strengthening healthy Qi and tonifying,dispelling wind and removing dampness,pr
