通窍活血汤对大鼠蛛网膜下腔出血后脑血管痉挛及氧化应激的影响

摘  要:目的 观察通窍活血汤对大鼠蛛网膜下腔出血后脑血管痉挛及氧化应激的影响。方法 将72只雄性SD大鼠按随机数字表法分为A组(假手术组)、B组(模型组)、C组(尼莫地平4.2 mg/kg)、D组(通窍活血汤低剂量6.3 g/kg)、E组(通窍活血汤高剂量12.6 g/kg)、F组(通窍活血汤12.6 g/kg+尼莫地平4.2 mg/kg)。采用枕大池二次注血法制作蛛网膜下腔出血大鼠模型,C组、D组、E组和F组予相应药物灌胃14 d,A组、B组采用等量生理盐水灌胃。评估各组大鼠神经功能评分;HE染色观察基底动脉血管形态变化,测量基底动脉管壁厚度、直径;酶联免疫吸附法检测脑组织中超氧化物歧化酶(SOD)、谷胱甘肽过氧化物酶(GSH-PX)、丙二醛(MDA)、一氧化氮(NO)、内皮素-1(ET-1)表达水平。结果 与A组比较,B组大鼠神经功能评分均显著下降,基底动脉管腔直径缩小、管壁厚度增加,脑组织中SOD、GSH-PX及NO水平显著下降,MDA含量及ET-1水平显著增加(P <0.01)。与B组比较,C、D、E、F组大鼠神经功能评分提高,基底动脉管腔直径增加、管壁厚度减少,脑组织中SOD、GSH-PX及NO水平提升,MDA含量及ET-1水平降低(P <0.05)。与D组比较,E、F组大鼠神经功能评分均提高,基底动脉管腔直径增加、管壁厚度减少,脑组织中SOD、GSH-PX及NO水平提升,MDA含量及ET-1水平降低。与C组比较,F组大鼠神经功能评分提高,基底动脉管腔直径增加、管壁厚度减少,脑组织中SOD、GSH-PX及NO水平提升,MDA含量及ET-1水平降低(P <0.05)。结论 通窍活血汤能抑制大鼠蛛网膜下腔出血后氧化应激反应,缓解脑血管痉挛(CVS),改善大鼠蛛网膜下腔出血后神经功能障碍和脑损伤。Objective:To observe the Effect of Tongqiao Huoxue Decoction on cerebral vasospasm(CVS)and oxidative stress in rats after subarachnoid hemorrhage(SAH).Methods:A total of 72 male SD rats were randomly divided into group A(sham operation group),group B(model group),group C(nimodipine group:4.2 mg/kg),group D(low-dose Tongqiao Huoxue Decoction group:6.3 g/kg),group E(high-dose Tongqiao Huoxue Decoction group:12.6 g/kg),and group F(Tongqiao Huoxue Decoction combined with nimodipine group:Tongqiao Huoxue Decoction 12.6 g/kg+nimodipine 4.2 mg/kg).A rat model of SAH was made using the double-injection method into the cisterna magna,and the rats were intervened with the medication by gavage for 14 consecutive days,while the sham operation group and the model group were given an equal volume of physiological saline by gavage.The neurologic function scores of the rats in each group were assessed;Hematoxylin-Eosin(HE)staining was used to observe the morphological changes of the basilar artery,and the thickness and diameter of the basilar artery wall were measured;the levels of superoxide dismutase(SOD),glutathione peroxidase(GSH-PX),malondialdehyde(MDA),nitric oxide(NO),and endothelin-1(ET-1)in brain tissue were detected by enzyme-linked immunosorbent assay(ELISA).Results:Compared with group A,the neurologic function scores of rats in group B were significantly decreased,the diameter of the basilar artery lumen was reduced,and the wall thickness increased;the levels of SOD,GSH-PX,and NO in brain tissue were significantly decreased,while the content of MDA and the level of ET-1 were significantly increased(P<0.01).Compared with group B,the neurologic function scores of rats in groups C,D,E,and F were improved,the diameter of the basilar artery lumen increased,and the wall thickness decreased;the levels of SOD,GSH-PX,and NO in brain tissue were elevated,and the content of MDA and the level of ET-1 were reduced(P<0.05).Compared with group D,the neurologic function scores of rats in groups E and F were improved,the diameter of
