开窍通络针刺法辅助西医治疗急性颅脑损伤术后患者的疗效观察

摘　　要：目的观察急性颅脑损伤术后患者应用开窍通络针刺法辅助西医治疗的疗效。方法回顾性分析98例急性颅脑损伤患者临床资料,以不同治疗手段分组,入组者均由同组医护人员行去骨瓣减压术治疗,对照组49例术后行西医治疗,研究组49例术后联合开窍通络针刺法治疗,对比两组临床疗效、血清学指标、神经功能、脑灌注压、颅内压及意识状态。结果研究组总有效率为95.92%,高于对照组的81.63%(P<0.05);研究组治疗5 d后、治疗12 d后颅内压、美国国立卫生研究院卒中量表(NIHSS)评分较对照组更低,脑灌注压、格拉斯哥昏迷量表(GCS)评分较对照组更高(P<0.05);两组治疗5 d后、治疗12 d后晚期蛋白氧化产物(AOPPs)、S-100B、神经元特异性醇化酶(NSE)水平较同组治疗前均降低,且研究组降低更明显(P<0.05)。结论急性颅脑损伤术后患者应用开窍通络针刺法辅助西医治疗,有利于疗效提高,可降低颅内压,并促进脑灌注压升高,有利于改善患者神经功能,并促进意识状态改善。Objective:To observe the therapeutic effect of applying the acupuncture method of activating orifices and dredging meridians to assist western medical treatment in postoperative patients with acute craniocerebral injury.Methods:A retrospective analysis was conducted on the clinical data of 98 patients with acute craniocerebral injury treated.Patients were grouped according to different treatment methods,with all participants undergoing decompressive craniectomy by the same medical team.The control group(49 cases)received western medical treatment after surgery,while the study group(49 cases)received a combination of acupuncture method of activating orifices and dredging meridians and western medicine after surgery.The clinical efficacy,serum indicators,neurological functions,cerebral perfusion pressure,intracranial pressure,and consciousness status were compared between the two groups.Results:The overall effective rate of the study group was 95.92%,which was significantly higher than 81.63%of the control group(P<0.05).Intracranial pressure and NIHSS scores of the study group were lower,while cerebral perfusion pressure and GCS scores were higher than those of the control group at 5 days and 12days after treatment(P<0.05).Levels of AOPPs,S-100B,and NSE decreased in both groups at 5 days and 12days after treatment compared with before treatment,with the decrease being more significant in the study group(P<0.05).Conclusion:Acupuncture method of activating orifices and dredging meridians in assisting western medicine for postoperative patients with acute craniocerebral injury contributes to improved efficacy,reduced intracranial pressure,enhanced cerebral perfusion pressure,improved neurological functions,and facilitated recovery of consciousness.
