解毒清火合剂治疗急性细菌性咽炎(肺胃热盛证)的前瞻临床对照研究

摘　　要：目的观察解毒清火合剂治疗急性细菌性咽炎(肺胃热盛证)的临床效果及对患者血清炎症指标的影响。方法100例患者按照块随机化原则分为对照组与观察组各50例,对照组采用常规治疗,观察组加用解毒清火合剂治疗。对比干预后的临床疗效,以及干预前后证候积分、炎症指标等变化情况。结果对照组痊愈率为68.00%,低于观察组的90.00%(P<0.05)。治疗后观察组视觉模拟量表(VAS)评分较对照组显著下调,简明疼痛调查表(BPI)评分则显著改善(P<0.01)。干预完成后,观察组患者咽干咽痛、口渴多饮、咽部充血、发热等主要症状评分较对照组显著下调(P<0.01);且观察组血清中白细胞介素-1β(IL-1β)、白细胞介素-6(IL-6)以及C反应蛋白(CRP)水平较对照组也显著下调(P<0.01)。干预过程中所有患者的主要不良反应有恶心呕吐、食欲不振、腹痛腹泻、皮疹等,两组患者的不良反应发生率无明显差异(P>0.05)。结论解毒清火合剂治疗急性细菌性咽炎(肺胃热盛证)具有良好的临床疗效,能够减轻患者咽痛、口渴多饮、咽部充血、发热等症状,作用途径与降低血清炎症因子水平、抑制炎症反应有关。Objective:To observe the clinical efficacy of Jiedu Qinghuo Mixture in the treatment of acute bacterial pharyngitis(syndrome of heat exuberance in lung and stomach)and its effect on patients′serum inflammation indicators.Methods:One hundred patients were divided into a control group and an observation group,50 patients each,according to the block randomization principle.The control group received antibiotic treatment,while the observation group was additionally treated with Jiedu Qinghuo Mixture.The clinical efficacy after intervention and changes in symptom scores and inflammation indicators before and after intervention were compared.Results:The cure rate in the control group was 68.00%,which was lower than 90.00%in the observation group(P<0.05).After treatment,the Visual Analogue Scale(VAS)score in the observation group was significantly lower than that in the control group,while the Brief Pain Inventory(BPI)score was significantly higher(P<0.01).After intervention,the observation group showed a significant decrease in scores for main symptoms such as sore throat,thirst,redness in the throat,and fever compared with the control group(P<0.01);furthermore,serum levels of IL-1β,IL-6,and CRP in the observation group were also significantly lower than in the control group(P<0.01).During the intervention,major adverse reactions in all patients included nausea,vomiting,loss of appetite,abdominal pain,diarrhea,and rash,with no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Jiedu Qinghuo Mixture demonstrates good clinical efficacy in the treatment of acute bacterial pharyngitis(syndrome of heat exuberance in lung and stomach),which is capable of alleviating symptoms such as sore throat,excessive thirst,redness in the throat,and fever.Its mechanism of action is related to reducing serum inflammatory factor levels and suppressing inflammatory responses.
