穴位埋揿针联合己酮可可碱对突发性耳聋患者凝血、免疫功能及内耳微循环的影响

摘　　要：目的观察穴位埋揿针联合己酮可可碱对突发性耳聋患者凝血、免疫功能及内耳微循环的影响。方法突发性耳聋患者100例采用随机数字表法分为对照组与研究组各50例。对照组予西医常规治疗,研究组在对照组治疗基础上予穴位埋揿针联合己酮可可碱治疗,比较两组临床疗效,记录两组眩晕消失时间、耳鸣消失时间、耳闷消失时间,检测听力损失程度、内耳道内听动脉、小脑前下动脉管径,检测免疫球蛋白G(IgG)、T淋巴细胞亚群CD4+、CD8+、凝血酶时间、纤维蛋白原、凝血酶原时间、内皮素-1(ET-1)、胱抑素C(CysC)、可溶性血管细胞间黏附分子-1(sVCAM-1)水平。结果治疗后,研究组总有效率高于对照组(P<0.05);研究组眩晕消失时间、耳鸣消失时间、耳闷消失时间均短于对照组(P<0.05);研究组患者听力损失程度均低于对照组(P<0.05);研究组内耳道内听动脉、小脑前下动脉管径大于对照组(P<0.05);研究组CD8+、ET-1、CysC、sVCAM-1、纤维蛋白原水平均低于对照组(P<0.05),研究组IgG、CD4+水平高于对照组(P<0.05);研究组凝血酶时间、凝血酶原时间长于对照组(P<0.05)。结论穴位埋揿针联合己酮可可碱治疗突发性耳聋,可有效改善血管内皮功能、免疫功能与凝血功能,减少听力损伤,改善耳内微循环,提高临床疗效。Objective:To explore the effects of acupoint embedding needle combined with pentoxifylline on coagulation,immune function,and inner ear microcirculation in patients with sudden deafness.Methods:Totally100 patients with sudden deafness were selected and randomly divided into a control group and a study group using random number table method,with 50 cases in each group.The control group was treated with conventional western medicine,while the study group was treated with acupoint embedding needle combined with pentoxifylline on the basis of the control group.The clinical efficacy of the two groups was compared,and the disappearance time of dizziness,tinnitus,and ear tightness were recorded.The degree of hearing loss,diameter of the internal auditory artery and anterior inferior cerebellar artery in the inner ear canal were measured,and immunoglobulin G(IgG),T lymphocyte subsets CD4+,CD8+,thrombin time,fibrinogen,prothrombin time,endothelin-1(ET-1),cystatin C(CysC),and soluble vascular cell adhesion molecule-1(sVCAM-1)levels were measured.Results:After treatment,the total effective rate of the study group was higher than that of the control group(P<0.05);the disappearance time of dizziness,tinnitus,and ear tightness in the study group were shorter than those in the control group(P<0.05);the degree of hearing loss in the study group was lower than that in the control group(P<0.05);the diameter of the auditory artery and anterior inferior cerebellar artery in the study group was larger than that in the control group(P<0.05);the average levels of CD8+,ET-1,CysC,sVCAM-1,and fibrinogen in the study group were lower than those in the control group(P<0.05),while the levels of IgG and CD4+in the study group were higher than those in the control group(P<0.05);the thrombin time and prothrombin time in the study group were longer than those in the control group(P<0.05).Conclusion:The combination of acupoint embedding needle and pentoxifylline can effectively improve vascular endothelial function,immune function,and coagulation
