开四关治疗呼吸性碱中毒临床经验总结

摘　　要：呼吸性碱中毒是临床中常见的内科疾病,尤以急诊内科常见,多发于中青年人群,以女性患者居多。西医治疗本病以对症治疗为主,多采取心理疏导、转移注意力、暗示疗法、镇静、解除痉挛等方法。笔者在多年中医临床工作中注重中医急救思维的运用,临证时力争在最短时间内做出诊断,给予早期干预,最大程度上延缓病情的进展。四关穴即双侧的合谷穴与太冲穴,开四关治疗呼吸性碱中毒可迅速缓解患者的临床症状,不仅可以减轻患者及家属的心理负担,且可减少用药的不良反应,安全性高,为中医适宜技术治疗本病提供新的临床思路。另外,针刺治疗具有双向调节作用,且无明显不良反应,在本病的治疗上有着独特的优势,值得临床参考运用。本病的发生与气机关系密切,涉及肝、肺、脾、胃,而四关穴是调整左右气机升降平衡的,凡是左升右降功能失常的疾病都可以采用开四关的方法治疗。笔者在传统开四关治疗的基础上经多次临床实践及研究,细化了四关穴针刺顺序以及通过一系列特定而有序的穴位补泻手法使上逆之气归于正常循环,从而达到“一气周流”、治疗疾病的目的,经临床验证确有疗效,值得进一步应用与推广。Respiratory alkalosis is a common internal medicine disease in clinical practice,especially in emergency medicine.RRespiratory alkalosis is common in middle-aged and young patients,mostly female patients.Western medicine mainly focuses on symptomatic treatment of this disease,using psychological counseling,attention diversion,suggestion therapy,sedation,and spasticity relief methods.In many years of clinical work in traditional Chinese medicine,the author has paid attention to the application of emergency thinking in traditional Chinese medicine,and strives to make a diagnosis in the shortest possible time and provide early intervention to postpone the progression of the disease.The Siguan(四关)acupoints,also known as the Hegu acupoints(LI4)and Taichong acupoints(LR03)on both sides,can be used to treat respiratory alkalosis.Opening the Siguan acupoints can quickly alleviate the clinical symptoms of patients,not only alleviate the psychological burden of patients and their families,but also reduce adverse reactions of medication.It has high safety and provides a new clinical approach for the appropriate treatment of this disease in traditional Chinese medicine.In addition,acupuncture treatment has a bidirectional adjustment effect and no obvious adverse reactions.It has its unique advantages in the treatment of this disease and is worthy of clinical reference and application.The author believes that the occurrence of this disease is closely related to the Qi(气)mechanism,involving the liver,lungs,spleen,and stomach.The Siguan acupoints are used to adjust the balance of left and right Qi movements,and can be used to treat most diseases with dysfunction of ascending and descending.Based on the traditional treatment of opening the Siguan points,the author has refined the acupuncture sequence of the Siguan points and the different tonifying and reducing techniques at each point through multiple clinical practices and studies.Through a series of specific and orderly acupuncture techniques,the upward and backward Qi
