电子胃镜取出失败的下咽部异物21例临床分析

摘　　要：目的总结经电子胃镜未能取出的下咽部异物患者的临床特点及诊治经验。方法回顾性分析自2014年2月~2022年3月在我院内镜中心电子胃镜下行下咽异物取出术未能成功的21例患者临床资料,总结其救治体会。结果21例患者中,1例异物已自行排出;1例异物迁移至颈部甲状腺内,经颈外侧径路成功取出异物;其余19例均在全麻下经食管镜成功取出异物。结论对于复杂的不规则形状下咽部异物并伴有局部软组织肿胀者,电子胃镜下取出异物困难时,传统的硬性食管镜手术仍然不失为一种有效的治疗方式;疗效。Objective To summarize the clinical characteristics of and treatment experience with hypopharyngeal foreign bodies failed to remove under electronic gastroscopy.Methods A retrospective was carried out among 21 patients with hypopharyngeal foreign bodies failed to remove under electronic gastroscopy in our Hospital from Feb.2014 to Mar.2022,with their clinical data carefully analyzed and our experience with the treatment in such cases reported.Results Among these cases,one was seen with the foreign body spontaneously discharged,one had the foreign body migrated into the adjacent tissue of thyroid gland and with the foreign body successfully removed through the operation of lateral cervical approach(a fish bone about 3.5cm long),and the other 19 cases were all with their foreign bodies successfully removed under classic rigid esophagoscopy.Conclusion The classic rigid esophagoscopy should be still an effective surgical procedure for treatment of some complex hypopharyngeal foreign bodies with an irregular shape,especially with obvious local soft tissue swelling in the field of a foreign body possibly present and being very difficult to remove under electronic gastroscope.
