基于“脾肾同治,水土合德”浅议肾病综合征的辨治

摘　　要：肾病综合征病情复杂、临床救治成功率低,中医药治疗本病有减毒增效、保护肾单位、减少并发症等优势。余兴权主任医师在30余年中医临床工作中总结了“脾肾同治,水土合德”辨治肾病综合征的学术思想,认为脾肾与肾病综合征的发病关系密切,脾肾互为依赖,先天肾水为体,后天脾土为用,二者互相影响,互根互用。治疗上重在调理脾肾,通过健脾助运化,温肾助气化,以运太阴为枢纽点、温少阴为动力点,实现水土合德,临床可获良效。并附验案一则,以资佐证。Nephrotic syndrome is complicated and the success rate of clinical treatment is low.Traditional Chinese medicine has the advantages of reducing toxicity and increasing efficiency,protecting renal units and reducing complications.Chief Physician Yu Xingquan summarized the academic thought of“treating spleen and kidney at the same time,combining water and soil”in the treatment of nephrotic syndrome in more than 30 years of clinical work of traditional Chinese medicine.He believed that spleen and kidney are closely related to the pathogenesis of nephrotic syndrome.Spleen and kidney depend on each other,congenital kidney water is the body,and acquired spleen soil is used.The two affect each other and interact with each other.The treatment focuses on regulating the spleen and kidney,through strengthening the spleen to promote transportation and transformation,warming the kidney to promote gasification,taking Taiyin as the hub point and warming Shaoyin as the power point to achieve the combination of water and soil,which can achieve good clinical results.A case is attached to support the evidence.
