促进还是抑制:转移支付对地方基本医疗卫生服务供给的影响研究

摘　　要：基本医疗卫生服务直接关乎民众健康和民生福祉,地方政府在基本医疗卫生服务供给中承担主要责任。作为弥补纵向财政失衡与缩小横向财力差距的政策工具,转移支付在促进地方基本医疗卫生服务供给方面具有重要作用。为此,本文立足中国式财政分权这一典型事实,基于30个省份面板数据构建固定效应模型,实证检验了转移支付对基本医疗卫生服务供给的影响。研究发现:转移支付制度总体上促进了地方基本医疗卫生服务供给,该结论经过一系列稳健性检验后依然成立。而且,这种正向促进作用存在显著的区域与转移支付结构异质性。鉴于此,指导部门应纵深推进财政体制改革、进一步完善转移支付制度、优化转移支付结构、综合施策以激励引导地方政府更加关注基本医疗卫生服务供给。Basic medical and health services are directly related to people's health and well-being,and local governments bear the main responsibility in the supply of basic medical and health services.As a policy tool to make up the vertical fiscal imbalance and narrow the horizontal financial gap,transfer payment plays an important role in promoting the supply of local basic medical and health services.On this account,based on the typical fact of Chinese fiscal decentralization,using the panel data of 30 provinces,this paper constructs a fixed effect model to empirically test the impact of transfer payments on the supply of basic medical and health services.The results show that the transfer payment system promotes the supply of local basic medical and health services in general,and this conclusion is still valid in a series of robustness tests.Moreover,this positive promotion effect has significant heterogeneity in region and transfer payment structure.In view of this,it is necessary to deepen the reform of the fiscal system,further improve the transfer payment system,optimize the transfer payment structure,and adopt comprehensive policies to encourage and guide local governments to pay more attention to the supply of basic medical and health services.
