摘 要:目的探讨徐州地区380例ABO正反定型不符的原因。方法整理2011年1月至2022年12月徐州市红十字血液中心输血研究科实验室为ABO正反定型不符的实验记录,通过分析受检者临床资料及ABO血型检测情况,对导致ABO正反定型不符的原因进行分类总结。结果共380例ABO正反定型不符记录,主要的原因包括自身抗体干扰151(39.74%)例,意外抗体干扰77(20.26%)例,ABO亚型77(20.26%)例,疾病导致的抗原减弱27(7.11%)例,蛋白凝集干扰19(5.00%)例;151例自身抗体干扰ABO正反定型中,导致额外抗原反应38(25.17%)例,导致额外抗体反应69(45.70%)例,导致额外的抗原反应和额外的抗体反应44例(29.14%);77例意外抗体干扰ABO正反定型中,MNS血型系统抗体42(54.55%)例,Rh血型系统抗体15(19.48%)例,P1Pk血型系统抗体3(3.90%)例,Lewis血型系统抗体1(1.30%)例,特异性不明的意外抗体16(20.80%)例。结论导致本地区ABO正反定型不符主要原因为自身抗体干扰、意外抗体干扰、ABO亚型,其中自身抗体能导致ABO正反定型不符表现出不同的类型,检测时要加以鉴别,保证ABO血型鉴定的准确性。Objective To investigate the reasons of forward and reverse type inconsistencies of 380 cases of ABO in Xuzhou area.Methods The experimental records of inconsistent positive and negative ABO typing were selected from Department of Laboratory Blood Transfusion Xuzhou Red Cross Blood Center from January 2011 to December 2022.According to analyzing the clinical data and ABO blood type detection of the subjects,the causes of inconsistent positive and negative ABO typing were classified and summarized.Results A total of 380 ABO positive and negative types were inconsistent.The main reasons included autoantibody interference in 151 cases(39.74%),accidental antibody interference in 77 cases(20.26%),ABO subtype in 77 cases(20.26%),disease-induced antigen reduction in 27 cases(7.11%),and protein agglutination interference in 19 cases(5.00%).In 151 cases,autoantibodies interfered with ABO positive and negative stereotyping,resulting in additional antigen reaction in 38(25.17%)cases,additional antibody reaction in 69(45.70%)cases,and additional antigen reaction and additional antibody reaction in 44(29.14%)cases.Among the 77 cases of accidental antibody interference with ABO positive and negative setting,there were 42(54.55%)MNS blood group system antibodies,15(19.48%)Rh blood group system antibodies,3(3.90%)P1Pk blood group system antibodies,and 1(1.30%)Lewis blood group system antibodies.There were 16 cases(20.80%)of accidental antibodies with unknown specificity.Conclusion The main causes of ABO positive and negative stereotyping inconsistency in this region are autoantibody interference,accidental antibody interference and ABO subtypes,among which autoantibodies can cause ABO positive and negative stereotyping inconsistency showing different types,which should be identified during detection to ensure the accuracy of ABO blood type identification.
