硝苯地平联合硫酸镁对重度妊娠期高血压患者尿激肽原、尿钙黏蛋白水平的影响

摘　　要：目的:研究硝苯地平联合硫酸镁对重度妊娠期高血压患者尿激肽原、尿钙黏蛋白水平的影响。方法:研究对象为120例2022年9月至2023年10月在本院治疗的重度妊娠期高血压患者,以随机数字表法分为对照组(n=60,采用硫酸镁治疗)及观察组(n=60,在对照组基础上采用硝苯地平治疗),比较两组患者的血压水平、血压恢复正常时间、尿蛋白水平、血管内皮功能、不良反应发生率及妊娠结局。结果:1)治疗1周后,两组患者血压水平均降低,且观察组较对照组低(P<0.05),观察组血压恢复正常时间较对照组短(P<0.05);2)治疗1周后,两组患者24h尿蛋白、尿钙黏蛋白及尿激肽原水平均降低,且观察组较对照组低(P<0.05);3)治疗1周后,两组患者一氧化氮均升高,且观察组更高(P<0.05),内皮素-1、血管内皮细胞钙黏蛋白均降低,且观察组更低(P<0.05);4)两组患者不良反应发生率(11.67%VS 13.33%)接近(P>0.05);5)随访结束后,观察组患者不良妊娠结局发生率(5.00%VS 16.67%)更低(P<0.05)。结论:硝苯地平联合硫酸镁可降低重度妊娠期高血压患者血压水平,促进其血压的恢复,同时降低其尿蛋白水平,提高其血管内皮功能,改善妊娠结局,安全性较高。Objective:To study the effect of nifedipine and magnesium sulfate on the levels of urokinin and urocadherin in patients with severe gestational hypertension.Methods:The study subjects were 120 patients with severe gestational hypertension treated in our hospital from September 2022 to October 2023,divided into control group(n=60,magnesium sulfate)and observation group(n=60,nifedipine in the control group)to compare the blood pressure level,blood pressure duration,urinary protein level,vascular endothelial function,incidence of adverse reactions and pregnancy outcome.Results:1)After 1 week of treatment,Lower blood pressure levels in both groups,And the observation group was lower than the control group(P<0.05),The recovery time of blood pressure in the observation group was shorter than that in the control group(P<0.05);2)After 1 week of treatment,In both groups,urinary protein,cadherin and urokininogen levels were decreased in 24h,Moreover,the observation group was lower than the control group(P<0.05);3)After 1 week of treatment,In both groups,And higher in the observation group(P<0.05),Endothelin-1,both have decreased cadherin in vascular endothelial cells,And it was lower in the observation group(P<0.05);4)The incidence of adverse reactions(11.67%VS 13.33%)in the two groups was close to that of patients(P>0.05);(5)After the completion of the follow-up visit,The incidence of adverse pregnancy outcomes(5.00%VS 16.67%)was lower in the observation group(P<0.05).Conclusion:Nifedipine and magnesium sulfate can reduce the blood pressure level,promote the recovery of their blood pressure,reduce the urinary protein level,improve the function.
