电针短刺腰部夹脊穴治疗慢性非特异性腰痛临床应用研究

摘　　要：目的:观察电针短刺腰部夹脊穴治疗慢性非特异性腰痛(CNLBP)临床应用研究。方法:选取2020年1月~2023年5月于本院接受治疗的CNLBP患者160例,以随机数字表法分为四组,每组40例。短刺组接受电针短刺夹脊穴治疗,夹脊组接受电针针刺夹脊穴治疗,普通电针组接受电针针刺常规配穴治疗,对照组予以口服药物治疗。对比四组临床治疗总有效率;比较四组治疗前后疼痛视觉模拟量表(VAS)评分及Oswestry功能障碍评分(ODI)评分;对比四组治疗过程中不良反应发生情况。比较四组治疗后复发率。结果:治疗4周后,短刺组治疗总有效率高于普通电针组及对照组(P<0.05)。治疗1周、2周、3周、4周后,短刺组VAS及ODI评分均低于夹脊组、普通电针组及对照组(P<0.05)。治疗期间,四组不良反应发生率无差异(P>0.05)。治疗后随访3个月、6个月,短刺组复发率低于夹脊组、普通电针组及对照组(P<0.05),夹脊组、普通电针组、对照组间复发率无差异(P>0.05)。结论:相较电针针刺夹脊穴、电针针刺常规配穴或口服药物治疗,电针短刺夹脊穴治疗CNLBP能提高疗效,更显著减轻患者疼痛不适,起效较快,复发率更低,具有较高的安全性。Objective:To observe the clinical application of short electroacupuncture at lumbar Jiaji acupoint in the treatment off chronic non-specific low back pain(CNLBP).Methods:A total of 160patients with CNLBP treated in the hospital were enrolled between January 2020to May 2023.According to random number table method,they were divided into short acupuncture group(short electroacupuncture at Jiaji acupoint),Jiaji group(electroacupuncture at Jiaji acupoint),common electroacupuncture group(electroacupuncture at routine acupoints)and control group(oral drugs),40cases in each group.The total response rate of clinical treatment,scores of visual analogue scale(VAS)and Oswestry disability index(ODI)before and after treatment,occurrence of adverse reactions during treatment and recurrence rate after treatment were compared among the four groups.Results:After 4weeks of treatment,total response rates of treatment in short acupuncture group were higher than common electroacupuncture group and control group(P<0.05),VAS and ODI scores were lower than those in the other three groups(P<0.05).During treatment,there was no difference in the incidence of adverse reactions among the four groups(P>0.05).After 3and 6months follow-up,recurrence rate in short acupuncture group was lower than that in the other three groups(P<0.05),but there was no significant difference among Jiaji group,common electroacupuncture group and control group(P>0.05).Conclusion:Compared with electroacupuncture at Jiaji acupoint,electroacupuncture at routine acupoints or oral drugs,short electroacupuncture at Jiaji acupoint can improve the curative effect on CNLBP patients,which can significantly relieve pain and discomfort,with faster onset,lower recurrence rate and higher safety.
