针刺运动疗法联合自拟蠲痹止痛汤治疗梨状肌综合征的临床效果

摘　　要：目的探讨针刺运动疗法联合自拟蠲痹止痛汤治疗梨状肌综合征的临床效果。方法将2022年8月至2023年10月于河南中医药大学第三附属医院就诊的79例梨状肌综合征患者按随机数字表法分对照组和研究组,给予对照组针刺运动疗法治疗,给予研究组针刺运动疗法联合自拟蠲痹止痛汤治疗,观察两组治疗前后的临床症状积分、麦吉尔疼痛问卷表(MPQ)评分、髋关节Harris评分和梨状肌厚度,评价两组临床疗效。结果治疗后,研究组临床症状积分、疼痛分级指数评分、视觉模拟疼痛评分、现时疼痛强度和MPQ总分低于对照组,髋关节Harris评分高于对照组(P<0.05)。治疗后研究组患侧梨状肌厚度低于对照组(P<0.05)。研究组的临床总有效率高于对照组(P<0.05)。结论针刺运动疗法联合自拟蠲痹止痛汤治疗梨状肌综合征的效果较好,能有效改善临床症状,缓解臀部疼痛,降低患侧梨状肌厚度,促进髋关节功能恢复,且安全性较好。Objective To investigate the clinical efficacy of acupuncture exercise therapy combined with self-prepared Juanbi Zhitong Decoction in the treatment of piriformis syndrome.Methods Seventy-nine patients with piriformis syndrome treated in the Third Affiliated Hospital of Henan University of Traditional Chinese Medicine from August 2022 to October 2023 were divided into control group and study group by random number table method.The control group was treated with acupuncture exercise therapy,while the study group was treated with acupuncture exercise therapy and self-prepared Juanbi Zhitong Decoction.Clinical symptom scores,McGill pain questionnaire(MPQ)score,hip Harris score and piriformis muscle thickness of the two groups before and after treatment were observed.The clinical effect of the two groups was evaluated.Results After treatment,the clinical symptom score,the pain grading index score,visual analogue scale,current pain intensity and MPQ total score of the study group were lower than those of the control group,and the hip Harris score was higher than that in the control group(P<0.05).The thickness of the affected piriformis muscle in the study group was lower than that in the control group after treatment(P<0.05).The total clinical effective rate of the study group was higher than that of the control group(P<0.05).Conclusion Acupuncture exercise therapy combined with self-formulated Juanbi Zhitong Decoction has good efficacy in the treatment of piriformis syndrome,which can effectively improve clinical symptoms,relieve hip pain,reduce the thickness of piriformis muscle on the affected side,and promote the functional recovery of hip joint with good safety.
