河南省南阳市宛城区2021-2023年初高中入学新生结核病筛查情况分析

摘　　要：目的分析2021—2023年河南省南阳市宛城区初高中入学新生结核病筛查情况,为进一步做好学校结核病防控工作提供依据。方法选取南阳市宛城区2021—2023年初高中入学新生作为研究对象,在所有学校新生入学第一学期开展结核菌素皮肤试验(TST),根据试验结果判断TST阳性率,采用描述流行病学方法对其流行特征进行分析。结果2021—2023年共调查57842名初高中入学新生,TST阳性10110例(17.48%),其中一般阳性203例(0.35%),中度阳性4173例(7.21%),重度阳性5734例(9.91%),2021年TST阳性率最高(20.55%)。3 a间TST阳性率(χ^(2)=266.425,P<0.01)差异有统计学意义,高中入学新生TST阳性率(20.72%)高于初中入学新生(χ^(2)=181.185,P<0.01),城区入学新生TST阳性率(18.88%)高于乡镇入学新生(χ^(2)=190.658,P<0.01),入学男生TST阳性率(19.73%)高于入学女生(χ^(2)=171.820,P<0.01)。结论河南省南阳市宛城区入学新生TST阳性率较高,学校结核病防治压力仍然较大,需要继续强化学校结核病防治措施的落实。Objective To analyze the screening of tuberculosis among junior and senior high school freshmen in Wancheng District,Nanyang City,Henan Province from 2021 to 2023,and to provide basis for further prevention and control of tuberculosis in schools.Methods Junior and senior high school freshmen enrolled in Wancheng District,Nanyang City from 2021 to 2023 were selected as the research subjects.The tuberculosis skin test(TST)was conducted in the first semester of enrollment for all new students.The positive rate of TST was determined based on the test results,and the epidemiological characteristics were analyzed using descriptive epidemiological methods.Results From 2021 to 2023,a total of 57842 new students enrolled in junior and senior high schools were surveyed.Among them,10110(17.48%)were TST positive,including 203(0.35%)of general positive cases,4173(7.21%)of moderate positive cases,and 5734(9.91%)of severe positive cases.The highest TST positive rate was in 2021(20.55%).There was a statistically significant difference in the TST positivity rate among 3 years(χ^(2)=266.425,P<0.01).The TST positivity rate of high school freshmen(20.72%)was higher than that of junior school freshmen(χ^(2)=181.185,P<0.01).The TST positivity rate of urban freshmen(18.88%)was higher than that of rural freshmen(χ^(2)=190.658,P<0.01).The TST positivity rate of male students(19.73%)was higher than that of female students(χ^(2)=171.820,P<0.01).Conclusion The positive rate of TST for junior and senior high school freshmen in Wancheng District,Nanyang City,Henan Province is high,and the pressure of school tuberculosis prevention and control is still high.It is necessary to continue to strengthen the implementation of school tuberculosis prevention and control measures.
