脑脊液IGF-1、β_(2)-MG、HBP与儿童化脓性脑膜炎发生发展的相关性

摘　　要：目的探究脑脊液中胰岛素样生长因子-1(IGF-1)、β_(2)微球蛋白(β_(2)-MG)、肝素结合蛋白(HBP)水平与儿童化脓性脑膜炎疾病发生发展的相关性。方法选取2021年2月至2023年2月郑州大学第一附属医院儿科收治的86例化脓性脑膜炎患儿为研究组,另选取86例非中枢神经系统感染患儿为对照组,比较两组脑脊液IGF-1、β_(2)-MG、HBP水平,探究其与研究组患儿病情严重程度及预后的相关性。结果研究组脑脊液中IGF-1、β_(2)-MG、HBP水平均高于对照组(P<0.05),且对化脓性脑膜炎具有较高的诊断价值,其曲线下面积分别为0.925、0.930、0.850;重度组脑脊液IGF-1、β_(2)-MG、HBP水平高于轻度组(P<0.05);预后不良组脑脊液IGF-1、β_(2)-MG、HBP水平高于预后良好组(P<0.05),且对化脓性脑膜炎预后不良具有较高的预测价值,其曲线下面积分别为0.879、0.854、0.822。结论监测化脓性脑膜炎患儿脑脊液中IGF-1、β_(2)-MG、HBP的水平对于早期诊断、判断病情严重程度以及预测疾病预后具有重要价值。Objective To explore the correlation of the expression level of insulin-like growth factor-1(IGF-1),β_(2)microglobulin(β_(2)-MG)and heparin binding protein(HBP)in cerebrospinal fluid and the occurrence and development of suppurative meningitis disease in children.Methods From February 2021 and February 2023,86 children diagnosed with purulent meningitis at the First Affiliated Hospital of Zhengzhou University were selected as the study group,and 86 children with non-central nervous system infection were selected as the control group.The expression of cerebrospinal fluid IGF-1,β_(2)-MG and HBP between the groups were compared,and the correlation with the severity and prognosis in the study group were explored.Results The cerebrospinal fluid levels of IGF-1,β_(2)-MG and HBP in the study group were higher than those in the control group(P<0.05).And they had high diagnostic value for suppurative meningitis,the area under the curve were 0.925,0.930,and 0.850,respectively.The levels of IGF-1,β_(2)-MG and HBP in the cerebrospinal fluid of the severe group were higher than those of the mild group(P<0.05).The levels of IGF-1,β_(2)-MG and HBP in cerebrospinal fluid of the poor prognosis group were higher than those of the good prognosis group(P<0.05).And they had high predictive value for the poor prognosis of suppurative meningitis,the area under the curve were 0.879,0.854,and 0.822,respectively.Conclusion Monitoring the level of IGF-1,β_(2)-MG and HBP in the cerebrospinal fluid of pediatric suppurative meningitis is important for early diagnosis,judging the severity of disease and prediction of disease prognosis.
