摘　　要：目的探究在原发性高血压患者心肌缺血诊断中采用动态心电图联合24h动态血压监测的价值.方法回顾性分析2022年1月-2023年12月500例原发性高血压患者心肌缺血临床资料,均对其进行常规心电图联合24h动态血压监测(设为对照组),动态心电图联合24h动态血压监测(设为观察组),以冠状动脉造影作为金标准,分析诊断价值.结果金标准、观察组、对照组确诊真阳性依次为254例、234例、169例.观察组对各级别原发性高血压患者心肌缺血的检出率较高(P<0.05).观察组灵敏度、特异度、阳性以及阴性预测值、诊断符合率较高(P<0.05).结论对原发性高血压患者心肌缺血诊断时,采用动态心电图联合24h动态血压监测,诊断价值较高.Objective To investigate the value of using dynamic electrocardiogram combined with 24-h ambulatory blood pressure monitoring in the diagnosis of myocardial ischemia in patients with essential hypertension.Methods The clinical data of 500 cases of myocardial ischemia in patients with essential hypertension from January 2022 to December 2023 were retrospectively analyzed,and they were all subjected to conventional electrocardiography combined with 24-h ambulatory blood pressure monitoring(set up as the control group),and dynamic electrocardiography combined with 24-h ambulatory blood pressure monitoring(set up as the observation group),and the diagnostic value of the diagnosis was analyzed by using coronary arteriography as the gold standard.Results The confirmed true-positive diagnosis of the gold standard,observation group,and control group was 254 cases,234 cases,and 169 cases in that order.The detection rate of myocardial ischemia in patients with primary hypertension at all levels was higher in the observation group(P<0.05).The sensitivity,specificity,positive as well as negative predictive value,and diagnostic compliance rate were higher in the observation group(P<0.05).Conclusion The diagnosis of myocardial ischemia in patients with essential hypertension is of high diagnostic value with the use of ambulatory electrocardiography combined with 24-h ambulatory blood pressure monitoring.
