低剂量甲氧明对妊娠合并糖尿病剖宫产腰硬联合麻醉产妇血流动力学及低血压风险的影响

摘　　要：目的研究低剂量甲氧明对妊娠合并糖尿病(GDM)剖宫产腰硬联合麻醉产妇血流动力学及低血压风险的影响.方法选择92例GDM产妇作为研究对象,均于2023年1月-2023年12月在青海红十字医院接受腰硬联合麻醉剖宫术,根据单双数法进行分组,单数分到观察组(n=46),双数分到对照组(n=46).麻醉取得预期效果后,对照组持续泵注盐酸甲氧明注射液2.5μg/kg/min,观察组持续泵注盐酸甲氧明注射液1.5μg/kg/min.观察和对比两组低血压发生率及围手术期血流动力学指标(HR、MAP、SVR、CO)变化.结果T_(0)、T_(1)两组HR、MAP、SVR、CO水平无统计学差异(P>0.05);T_(2)、T_(3)、T_(4)两组HR、MAP、SVR、CO水平降低,但对照组更低(P<0.05).观察组低血压发生率为2.17%,显著低于对照组的17.39%(P<0.05).结论对GDM剖宫产腰硬联合麻醉产妇应用低剂量甲氧明,对血流动力学的影响较轻微,且能有效降低低血压风险.Objective To study and analyze the effect of low-dose methoxamine on hemodynamics and hypotension risk in pregnant women with diabetes(GDM)undergoing cesarean section under combined spinal epidural anesthesia.Methods 92 GDM pregnant women were selected as the study subjects,all of whom underwent combined spinal and epidural anesthesia cesarean section in our hospital from January 2023 to December 2023.They were divided into an observation group(n=46)and a control group(n=46)according to the odd and even number method.After achieving the expected effect of anesthesia,the control group continued to pump with Methoxymine Hydrochloride Injection 2.5μg/kg/min,the observation group was continuously pumped with Methoxymine Hydrochloride Injection 1.5μg/kg/min.Observe and compare the incidence of hypotension and changes in perioperative hemodynamic indicators(HR,MAP,SVR,CO)levels between the two groups.Results There was no statistically significant difference in HR,MAP,SVR,and CO levels between the T_(0) and T_(1) groups(P>0.05);the levels of HR,MAP,SVR,and CO decreased in the T_(2),T_(3),and T_(4) groups,but were lower in the control group(P<0.05).The incidence of hypotension in the observation group was 2.17%,significantly lower than 17.39%in the control group(P<0.05).Conclusion The application of low-dose methoxamine in GDM cesarean section with lumbar epidural anesthesia has a mild impact on hemodynamics and can effectively reduce the risk of hypotension.
