化肝煎合左金丸加减治疗胃溃疡的应用及有效性评价

摘　　要：目的探究胃溃疡患者采取化肝煎合左金丸加减治疗的临床有效性.方法此次研究对象抽取自无锡市锡山区云林街道社区卫生服务中心收治的120例胃溃疡患者.采用随机数字表法将研究对象均分成两组,即参比组和研究组,选取时间段为2023年1月-2023年12月.予以参比组常规西医治疗,予以研究组化肝煎合左金丸加减治疗.对两组的临床疗效、胃黏膜屏障功能、溃疡面积、炎症情况、胃功能、不良反应情况、Hp清除率、复发率进行比对.结果治疗后,研究组治疗总有效率比参比组高(P<0.05).与治疗前相比,治疗后两组黏膜厚度评分、腺体密度评分均降低,且研究组比参比组低;溃疡面积均减少,且研究组比参比组少(P<0.05).与治疗前相比,治疗后两组TNF-α、CRP、IL-6水平均降低,且研究组比参比组低(P<0.05).与治疗前相比,治疗后两组GAS、CCK均减少,且研究组比参比组少;MTL均增多,且研究组比参比组多(P<0.05).两组不良反应发生率相比,差异不显著(P>0.05).随访6个月后,研究组Hp清除率比参比组高,复发率比参比组低(P<0.05).结论针对胃溃疡患者采取化肝煎合左金丸加减治疗效果确切,可提高患者整体疗效,促进患者胃功能、炎症情况改善,并可减少不良反应与复发几率,安全性较高,值得临床大力推广.Objective To explore the clinical efficacy of Huaganjian combined with Zuojin Pill in the treatment of gastric ulcer.Methods 120 patients with gastric ulcer were selected from our hospital.All subjects were randomly divided into two groups,60 in the reference group and 60 in the study group.The time period was from January to December in the 2023.The reference group was treated with routine Western medicine,and the study group was treated with Huaganjian and Zuojin Pill.The clinical efficacy,gastric mucosal barrier function,ulcer area,inflammation,gastric function,adverse reaction,HP clearance rate and recurrence rate were compared between the two groups.Results The total effective rate of the study group was higher than that of the reference group(P<0.05).After treatment,the scores of mucosal thickness and glandular density in both groups were lower than those in the reference group,and the area of ulcer in the study group was lower than that in the reference group(P<0.05).After treatment,the levels of TNF-α,CRP and IL-6 in the two groups were lower than those before treatment,and the levels of TNF-α,CRP and IL-6 in the study group were lower than those in the reference group(P<0.05).After treatment,the levels of GAS and CCK in both groups were decreased,and the levels of MTL were increased,and the levels of GAS and CCK in the study group were higher than those in the reference group(P<0.05).The incidence of adverse reactions in the two groups was compared(P>0.05).After 6-month follow-up,the clearance rate of HP was higher and the recurrence rate was lower in the study group than in the reference group(P<0.05).Conclusion The curative effect of Huaganjian combined with Zuojin Pill on gastric ulcer is definite,which can improve the whole curative effect,promote the improvement of gastric function and inflammation,and reduce the rate of adverse reaction and recurrence,it has high safety and is worth to be popularized and applied in clinic.
