基于文献计量分析桥接数据挖掘探讨近现代中医名家治疗类风湿关节炎的用药规律

摘　　要：目的:探析近现代名中医治疗类风湿关节炎(RA)经验的研究现状、热点和用药规律,为RA现代临床研究提供参考。方法:检索中国知识资源总库(CNKI)、中国学术期刊数据库(万方)、维普中文期刊服务平台(VIP)、中国生物医学文献数据库(CBM)1993年1月1日—2023年8月8日收录的名中医治疗RA经验的文献,采用Excel、VOSviewer及CiteSpace进行发文量、作者、机构、期刊、关键词共现、爆发分析并绘制知识图谱。提取名中医治疗RA的医案信息,建立数据库,Excel、SPSS Modeler、OriginPro及CytoScape软件用于中药频次统计、关联规则、聚类分析及复杂网络分析。结果:共纳入1037篇文献,该领域发文量整体呈上升趋势;共涉及1988位作者,形成以刘健、旷惠桃、彭江云、高明利等为代表的多个研究团队,研究机构以南京中医药大学发文最多,数据挖掘、用药规律为研究热点。共纳入530首完整处方,涉及344味中药,高频药物包括桂枝、白芍、当归、黄芪、威灵仙等,以补虚药、祛风湿药为主,药性温性最多,药味以甘味为主,多归于肝、脾经;关联规则分析得到13条二项关联规则,16条三项关联规则;聚类分析得到3首新处方;复杂网络分析得到24个核心药物。结论:近30年名中医治疗RA经验研究蓬勃发展,基于数据挖掘技术的药对、用药规律挖掘成为热门话题和研究趋势;名中医治疗RA用药以益气养血补虚配合祛风除湿为主,兼顾活血化瘀、清热通络。Objective:To explore the current research status,hotspots and medication rules of the treatment of rheumatoid arthritis(RA)by TCM masters in modern times,and provide references for modern clinical research of RA.Meth⁃ods:The literature on the experience of TCM masters in the treatment of RA collected from Chinese National Knowledge Infrastructure(CNKI),China Academic Journal Database(Wanfang),China Science and Technology Journal Database(VIP)and China Biology Medicine Disc(CBM)since 1993 were searched.Excel,VOS viewer and CiteSpace were used to analyze the number of publications,authors,institutions,journals,keyword co-occurrence and outbreak,and draw the mapping knowledge domain.The medical records of RA patients treated with TCM masters were extracted and a database was established.Excel,SPSS Modeler,OriginPro and Cytoscape software were used for Chinese herbal frequency statistics,association rules,cluster analysis and complex network analysis.Results:Literature measurement included 1037 literature,and the number of published papers in this field showed an overall upward trend.A total of 1988 authors were involved,forming a number of research teams represented by Liu Jian,Kuang Huitao,Peng Jiangyun,Gao Mingli,etc.Nanjing University of Chinese Medicine published the most papers,and data mining and medication rules were the research hotspots.A total of 530 complete prescriptions involving 344 Chinese herbs were included.High frequency drugs included Guizhi,Baishao,Danggui,Weilingxian,and so on.The efficacy of the Chinese herbs was mainly used for tonifying deficiency and dispelling wind-dampness.The nature and flavor of the Chinese herbs were mainly sweet,bitter,pungent and warm,and these herbs were attributed to the liver and spleen meridians.The analysis of association rules obtained 13 binomial associations and 16 ternary associations.Cluster analysis yielded three prescriptions.The complex network analysis obtained 24 core drugs.Con⁃clusion:In the past 30 years,the research on the treatment of RA by TCM
