摘  要:目的观察3D打印技术辅助内外侧锁定钢板内固定治疗复杂胫骨平台骨折的临床效果。方法选择2020年6月至2022年6月收治的80例复杂胫骨平台骨折患者作为研究对象,采用随机数字表法将其分为对照组和观察组,各40例。两组均给予内外侧锁定钢板内固定治疗,对照组根据术前影像检查结果及术者经验完成手术,观察组采用3D打印技术辅助完成手术。比较两组的治疗效果。结果观察组的手术时间、骨折愈合时间及完全负重时间短于对照组,术中出血量、透视次数少于对照组(P<0.05)。术后1、3、6个月,观察组的Rasmussen放射学评分高于对照组(P<0.05)。术后1、3个月,观察组的胫骨平台内翻角、胫骨平台后倾角大于对照组(P<0.05)。术后1、3个月,观察组的美国特种外科医院膝关节评分(HSS)、Fugl-Meyer运动功能评定量表(FMA)评分高于对照组(P<0.05)。结论3D打印技术辅助内外侧锁定钢板内固定治疗复杂胫骨平台骨折可提高手术效果,还有利于提高骨折复位质量,值得临床推广。Objective To observe the clinical effect of 3D printing technology assisted medial and lateral locking plate internal fixation in the treatment of complex tibial plateau fractures.Methods A total of 80 patients with complex tibial plateau fractures admitted from June 2020 to June 2022 were selected as the research objects.The patients were divided into control group and observation group by random number table method,with 40 cases in each group.Both groups were treated with medial and lateral locking plate internal fixation,the control group completed the operation according to the results of preoperative imaging examination and the experience of the surgeon,and the observation group used 3D printing technology to assist in the operation.The therapeutic effects of the two groups were compared.Results The operation time,fracture healing time and full weight-bearing time of the observation group were shorter than those of the control group,and the intraoperative blood loss and fluoroscopy times were less than those of the control group(P<0.05).At 1,3 and 6 months after operation,the Rasmussen radiological score of the observation group was higher than that of the control group(P<0.05).At 1 and 3 months after operation,the tibial plateau varus angle and tibial plateau posterior slope angle in the observation group were greater than those in the control group(P<0.05).At 1 and 3 months after operation,the Hospital for Special Surgery Knee Score(HSS)and Fugl-Meyer Assessment(FMA)scores of the observation group were higher than those of the control group(P<0.05).Conclusion 3D printing technology assisted medial and lateral locking plate internal fixation in the treatment of complex tibial plateau fractures can improve the surgical effect,but also help to improve the quality of fracture reduction,which is worthy of clinical promotion.
