生脉散合六味地黄汤加减治疗气阴两虚型早期2型糖尿病肾病临床研究

摘　　要：目的:观察生脉散合六味地黄汤加减治疗气阴两虚型早期2型糖尿病肾病的临床疗效。方法:选取60例气阴两虚型早期2型糖尿病肾病患者,以随机数字表法分为观察组与对照组各30例。对照组给予常规西药治疗,观察组在对照组基础上给予生脉散合六味地黄汤加减治疗。比较2组临床疗效、中医证候评分、肾功能指标水平及炎性因子水平。结果:观察组总有效率93.33%,高于对照组70.00%(P<0.05)。治疗后,2组各项中医证候评分均较治疗前降低(P<0.05),观察组各项中医证候评分均低于对照组(P<0.05)。治疗后,2组内生肌酐清除率(CCr)均较治疗前升高(P<0.05),观察组CCr高于对照组(P<0.05);2组尿蛋白/肌酐比值(UACR)均较治疗前降低(P<0.05),观察组UACR低于对照组(P<0.05)。治疗后,2组血清降钙素原(PCT)、超敏C-反应蛋白(hs-CRP)水平均较治疗前降低(P<0.05),观察组血清PCT、hs-CRP水平均低于对照组(P<0.05)。结论:生脉散合六味地黄汤加减治疗气阴两虚型早期2型糖尿病肾病可提高临床疗效,减轻临床症状,改善肾功能,调节机体炎性反应。Objective:To observe the clinical effect of modified Shengmai San combined with Liuwei Dihuang Decoction in early stage of type 2 diabetes nephropathy of qi-yin deficiency type.Methods:A total of 60 cases of patients in early stage of type 2 diabetes nephropathy of qi-yin deficiency type were selected and divided into the observation group and the control group according to the random number table method,with 30 cases in each group.The control group was treated with conventional western medicine,and the observation group was additionally treated with modified Shengmai San combined with Liuwei Dihuang Decoction based on the treatment of the control group.Clinical effects,Traditional Chinese medicine(TCM)syndrome scores,kidney function indexes levels,and inflammatory factors levels were compared between the two groups before and after treatment.Results:The total effective rate was 93.33%in the observation group,higher than that of 70.00%in the control group(P<0.05).After treatment,the various TCM syndrome scores in the two groups were decreased when compared with those before treatment(P<0.05),and the various TCM syndrome scores in the observation group were lower than those in the control group(P<0.05).After treatment,creatinine clearance rates(CCr)in the two groups were increased when compared with those before treatment(P<0.05),and CCr in the observation group was higher than that in the control group(P<0.05);urine albumin-creatinine ratio(UACR)in the two groups was decreased when compared with those before treatment(P<0.05),and UACR in the observation group was lower than that in the control group(P<0.05).After treatment,the levels of procalcitonin(PCT)and hypersensitive C-reactive protein(hs-CRP)in serum in the two groups were decreased when compared with those before treatment(P<0.05),and the levels of PCT and hs-CRP in serum in the observation group were lower than those in the control group(P<0.05).Conclusion:Modified Shengmai San combined with Liuwei Dihuang Decoction can improve the clinical effects on pa
