张念志运用“轻可去实”“宣可去壅”法治疗急性支气管炎临证撷英

摘　　要：文章通过对“轻可去实”“宣可去壅”法(以下简称“轻宣之法”)的介绍,对急性支气管炎病因病机的分析,运用轻宣法治疗不同阶段急性支气管炎的阐述及三则医案记录总结了张念志教授运用轻宣之法治疗急性支气管炎的临证经验,张教授认为轻宣之法包括治上焦如羽,非轻莫举;轻可去实,宣可去壅;培土生金,既病防变三个方面;在治疗上针对急性支气管炎的病因病机合理应用。急性支气管炎初期外邪侵袭,邪浅病轻,用药清灵;中期肺气郁闭,肺主行水功能障碍,痰湿蕴肺,蕴久化热,肺失宣降,腑气不通则宣肺开闭,清热祛痰,理气通腑;恢复期子病及母,脾失健运,肺脾同病则在培土生金的同时遵轻宣之法梳理气机,补而不滞。临床根据不同的病程阶段和患者的症状表现灵活化裁,取得良好的治疗效果。Acute bronchitis is an acute inflammation of the tracheal and bronchial mucosa caused by infection,physical and chemical stimulation,allergy and other factors.This article introduces the methods of“Xiaoke Removing Excess”and“Xuanke Removing Obstructing”(hereinafter referred to as“Xiaoxuan method”),and analyzes the etiol-ogy and pathogenesis of acute bronchitis.ZHANG Nianzhi’s clinical experience in the treatment of acute bronchitis by using the method of light propaganda is summarized in the exposition of different stages of acute bronchitis and three medical re cords.Light can go to reality,Xuan can go to obstructing;Cultivate soil to produce gold,both disease pre-vention three aspects.In the treatment of acute bronchitis for the etiology and pathogenesis of reasonable application.In the early stage of acute bronchitis,the invasion of external evil,shallow evil disease,drug Qingling;Intermediate lung Qi stag nation,lung main line water dysfunction,phlegm dampness of the lung,accumulated heat,lung loss of procla-mation,Fu-Qi is not open and closed,clearing heat expectorant,regulating Qi Tongfu;Convalescent child disease and mother;spleen loss of health transport,lung and spleen with disease is in the cultivation of soil and gold at the same time follow the light Xuan method of combing Qi machine,fill without stagnation.According to the different stages of the disease and the symptoms of the patient,it is flexible and has achieved good therapeutic effect.
