摘  要:目的探讨孕晚期阴道微生态失调对新生儿发生败血症造成的影响,为做好新生儿败血症的预防和治疗提供理论基础。方法本研究选取2020年6月至2023年6月在我院接受治疗的50例败血症患儿的母亲作为研究组,随机选择50名正常新生儿的母亲作为对照组。通过比较两组研究对象孕晚期阴道微生态的变化,探讨其与新生儿败血症之间的关系。结果研究组阴道微生态失调率比对照组高(P<0.05);研究组革兰阴性杆菌阳性率比对照组低,念珠菌阳性率比对照组高(P<0.05)。经过Logistic回归分析发现,孕晚期念珠菌阳性与败血症发病率有显著的正相关性(OR 3.721,P=0.036)。在评价新生儿败血症发病时,念珠菌阳性的ROC曲线下面积(AUC)为0.527。结论临床上应注意孕妇在孕晚期发生阴道微生态紊乱的情况,及时有效的干预有助于减少新生儿败血症的发生。Objective To explore the effect of vaginal microecological imbalance on neonatal sepsis in the third trimester,and to provide a theoretical basis for the prevention and treatment of neonatal sepsis.Methods In this study,mothers of 50 children with sepsis who were treated in our hospital from June 2020 to June 2023 were selected as the study group,and 50 mothers of normal newborns were randomly selected as the control group.By comparing the changes of vaginal microecology in late pregnancy between the two groups,the relationship between the changes and neonatal sepsis was discussed,and the role of the changes in neonatal sepsis was discussed.Results The rate of vaginal microecological disorder in the study group was higher than that in the control group(P<0.05).The positive rate of gram-negative bacteria in the study group was lower than that in the control group,and the positive rate of candida was higher than that in the control group(P<0.05).Logistic regression analysis showed that there was a significant positive correlation between candida positive in late pregnancy and the incidence of septicemia(OR 3.721,P=0.036).In evaluating the onset of neonatal sepsis,the area under the ROC curve(AUC)of candida-positive was 0.527.Conclusions Clinical attention should be paid to vaginal microecological disorders in pregnant women in the third trimester,and timely and effective intervention can help reduce the occurrence of neonatal sepsis.
