期待治疗对未足月胎膜早破患者妊娠结局的影响

摘　　要：目的观察期待治疗对未足月胎膜早破患者妊娠结局的影响。方法本次研究共纳入114例患者,均为我院2020年10月至2023年2月接受期待治疗的未足月胎膜早破患者,根据患者入院时的孕周进行分组:将孕28~31+6周设为早期组(36例),将孕32~33+6周设为中期组(40例),将34~36+6设为晚期组(38例)。对比3组患者的妊娠延长时间,产妇出血量,新生儿相关指标(新生儿体重、Apgar评分),新生儿并发症发生情况以及不良妊娠结局。结果早期组、中期组、晚期组的妊娠延长时间、期待治疗时间以及产妇出血量均呈递减趋势,中期组的妊娠延长时间、期待治疗时间短于早期组,出血量少于早期组,晚期组妊娠延长时间、期待治疗时间短于中期组,出血量少于中期组(均P<0.05)。新生儿Apgar评分与孕周正相关,中期组新生儿出生后5 min的Apgar评分高于早期组,晚期组新生儿出生后5 min的Apgar评分高于中期组(均P<0.05)。中期组和晚期组的新生儿并发症发生率均低于早期组(P<0.05),中期组和晚期组相比差异不大(P>0.05)。中期组和晚期组的不良妊娠结局发生率均低于早期组(P<0.05),中期组和晚期组相比差异不大(P>0.05)。结论为不同孕周未足月胎膜早破患者应用期待治疗后,孕周<32周的产妇效果有限,而孕周在32~36周范围内效果较为理想,可有效改善妊娠结局,降低新生儿并发症的发生风险。Objective To observe the effect of expectant treatment of premature rupture of membranes in different gestational weeks on pregnancy outcome.Methods A total of 114 patients were included in this study,all of whom were preterm patients with premature rupture of membranes who received expectational treatment in our hospital from October 2020 to February 2023.The patients were grouped according to their gestational weeks at admission:The patients were selected as the early group(36 cases)at 28 to 31+6 weeks of gestation,the middle group(40 cases)at 32 to 33+6 weeks of gestation,and the late group(38 cases)at 34 to 36+6 weeks of gestation.The pregnancy extension time,maternal blood loss,neonatal related indexes(neonatal body mass,Apgar score),neonatal complications and adverse pregnancy outcomes were compared among the three groups.Results The duration of pregnancy extension,expectant treatment time,and the amount of maternal bleeding in the early,middle,and late groups all show a decreasing trend.The duration of pregnancy extension and expectant treatment time in the middle group are shorter than those in the early group,with less bleeding.The duration of pregnancy extension and expectant treatment time in the late group are shorter than those in the middle group,with less bleeding(P<0.05).Apgar score was positively correlated with gestational week.Apgar score 5min after birth in the middle stage group was higher than that in the early stage group,and Apgar score 5min after birth in the late stage group was higher than that in the middle stage group(all P<0.05).The incidence of neonatal complications in the middle and late groups was significantly lower than that in the early group(P<0.05),and there was no significant difference between the middle and late groups(P>0.05).The incidence of adverse pregnancy outcomes in the middle and late groups was significantly lower than that in the early group(P<0.05),and there was no significant difference between the middle and late groups(P>0.05).Conclusions Expectant treatment
