2021—2023年福清市健康体检人员甲肝、戊肝病毒IgM抗体筛查的结果分析  

Analysis of Screening Results for IgM Antibodies Against Hepatitis A and Hepatitis E Virus Among Health Examination Personnel in Fuqing City from 2021 to 2023

摘　　要：目的针对2021—2023年到本中心健康体检人员甲肝、戊肝病毒IgM抗体(HAV-IgM、HEV-IgM)筛查情况进行回顾分析。方法选择2021年1月至2023年12月到福清市疾病预防控制中心进行健康体检的食品或公共场所从业人员(受试者),采用酶联免疫吸附试验(ELISA)检测HAV-IgM及HEV-IgM,全自动生化分析仪检测肝功能指标丙氨酸转氨酶(ALT)水平。对检测结果进行回顾分析。结果结合统计2021—2023年体检情况,不同年度进行健康体检人数分别为33510人、24826人、30942人,其中HAV-IgM阳性分别为14例、36例、12例,HEV-IgM阳性分别为33例、78例、25例,HAV-IgM、HEV-IgM均阳性为0例、2例、0例。HAV-IgM阳性检出率分别为0.04%、0.15%、0.04%,HEV-IgM阳性检出率分别为0.10%、0.34%、0.08%。18~30岁受试者HAV-IgM、HEV-IgM阳性率分别为0.06%、0.13%、31~50岁为0.08%、0.17%,>50岁为0.07%、0.16%。2021—2023年阳性病例中丙氨酸转氨酶(ALT)异常率,分别为21.25%、17.24%、21.62%。结论HAV-IgM、HEV-IgM抗体阳性率均相对较低,HAV阳性率低于HEV阳性率。阳性病例年龄分布以31~50岁群体为主。丙氨酸转氨酶(ALT)指标异常水平与HAV-IgM、HEV-IgM抗体阳性率相关性不强。阳性病例吸光度值低,其抗体的表达水平不高。Objective To conduct a retrospective analysis of hepatitis A and hepatitis E virus IgM antibody(HAV-IgM,HEV-IgM)screening for people who came to our center for health checkups in 2021-2023.Methods Food or public place workers(subjects)who went to the Fuqing Center for Disease Control and Prevention for health checkups from January 2021 to December 2023 were selected to detect HAV-IgM and HEV-IgM by enzyme-linked immunosorbent assay(ELISA),and the level of alanine aminotransferase(ALT),an indicator of liver function,by fully automated biochemical analyzer.The test results were retrospectively analyzed.Results Combined with the statistics of physical examination from 2021 to 2023,the number of people who underwent physical examination in different years was 33510,24826 and 30942,among which 14 cases,36 cases and 12 cases were positive for HAV-IgM and 33 cases,78 cases and 25 cases were positive for HEV-IgM,respectively.HAV-IgM and HEV-IgM were positive in 0 cases,2 cases and 0 cases.The positive detection rates of HAV-IgM and HEV-IgM were 0.04%,0.15%,0.04%,and 0.10%,0.34%,0.08%,respectively.The positive rates of HAV-IgM and HEV-IgM were 0.06%and 0.13%in 18-30 years old,0.08%and 0.17%in 31-50 years old,and 0.07%and 0.16%in>50 years old,respectively.The abnormal rates of alanine aminotransferase(ALT)in positive cases from 2021 to 2023 were 21.25%,17.24%and 21.62%,respectively.Conclusions The IgM antibody positivity rates for both hepatitis A and E viruses were relatively low,with hepatitis A positivity lower than hepatitis E positivity.The age distribution of positive cases was dominated by the group of 31-50 years old.The correlation between the abnormal levels of alanine aminotransferase(ALT)and the positive rates of IgM antibodies against hepatitis A and E viruses was not strong.Positive cases had low absorbance values and their antibody expression levels were not high.
