摘　　要：目的系统评价老年心力衰竭患者衰弱现状及其影响因素。方法计算机检索从建库-2022年10月PubMed、EmBase、The Cochrane Library、Web of Science、中国知网、万方、维普、中国生物医学文献数据库,收集有关老年心力衰竭患者衰弱发生影响因素的研究,采用Stata16.0进行meta分析。结果纳入15篇文献,共2856例心力衰竭患者,meta分析结果显示,老年心力衰竭患者衰弱患病率为43%(95%CI:0.35-0.51);影响心衰患者衰弱的各因素OR值及(95%CI)分别为NYHA心功能分级4.11(2.92-5.80)、多重用药2.72(1.61-4.59)、Charlson合并症指数1.25(1.08-1.46)、住院时间0.81(0.74-0.89)、近一年跌倒史0.16(0.07-0.36)。结论老年心衰患者衰弱患病率较高,影响因素较多,有必要开展护理干预改善老年心衰患者衰弱现状。Objective To systematically evaluate the frailty status of elderly patients with heart failure and its influencing factors.Methods Literature retrieved from PubMed,EmBase,The Cochrane Library,Web of Science,China National Knowledge Infrastructure(CNKI),Wan fang,VIP,and Chinese Biomedical Literature Database(CBM)from the establishment to July 2022 were collected to analyze influencing factors of frailty in elderly patients with heart failure,and then meta-analysis was performed using Stata16.0.Results Fifteen papers with 2856 heart failure patients were included,and the results of meta-analysis showed that the prevalence of frailty in elderly patients with heart failure was 43%(95%CI:0.35-0.51,P<0.01).The ORs and 95%CI of the influencing factors were NYHA cardiac function grade 4.11(2.92-5.80),multidrug grade 2.72(1.61-4.59),Charlson comorbidity index 1.25(1.08-1.46),length of hospital stay 0.81(0.74-0.89),fall history in the last year 0.16(0.07-0.36).Conclusion The prevalence of frailty in elderly patients with heart failure is relatively high,and there are many influencing factors,so it is necessary to carry out nursing interventions to improve the current status of frailty in elderly patients with heart failure.
