救肝开郁汤方联合佐匹克隆治疗老年慢性不寐临床研究

摘　　要：目的:探讨救肝开郁汤方联合佐匹克隆治疗老年慢性不寐患者的临床疗效。方法:回顾性纳入2022年1月~2023年1月在我院就诊的165例老年慢性不寐患者进行研究。按照治疗方法的不同将上述患者中使用佐匹克隆的85例患者设为对照组,使用救肝开郁汤方联合佐匹克隆治疗的80例患者设为联合组。比较两组患者的临床疗效,治疗前后的中医症状积分、睡眠质量[匹兹堡睡眠质量指数量表(PSQI)]、睡眠结构(睡眠总时间、睡眠潜伏期、觉醒时间)、血清指标(MT、DA)以及治疗过程中的不良反应发生情况。结果:联合组患者的治疗有效率为91.25%,高于对照组患者的治疗有效率76.47%(P<0.05);治疗前,两组患者的各中医症状积分、PSQI评分、睡眠总时间、睡眠潜伏期、觉醒时间、MT、DA比较差异无统计学意义(P>0.05)。治疗后,联合组患者中医症状积分、PSQI评分、睡眠潜伏期、觉醒时间均低于对照组患者(P<0.05);治疗后,联合组患者睡眠总时间、DA高于对照组患者(P<0.05);联合组患者的不良反应率为7.50%低于对照组患者的不良反应率20.93%(P<0.05)。结论:救肝开郁汤方联合佐匹克隆治疗老年慢性不寐患者疗效确切,可有效改善患者睡眠质量,调节神经中枢神经递质。Objective:To explore the clinical curative effect of Jiugan Kaiyu Decoction combined with zopiclone in elderly patients with chronic insomnia.Methods:A total of 165elderly patients with chronic insomnia in the hospital were retrospectively enrolled between January 2022and January 2023.According to different treatment methods,they were divided into control group(85cases,zopiclone)and combination group(80cases,Jiugan Kaiyu Decoction combined with zopiclone).The clinical curative effect,scores of TCM symptoms,sleep quality[Pittsburgh Sleep Quality Index(PSQI)],sleep structure(total sleep time,sleep latency,awakening time)and serum indexes[melatonin(MT),dopamine(DA)]before and after treatment,and the occurrence of adverse reactions during treatment were compared between the two groups.Results:The response rate of treatment in combination group was higher than that in control group(91.25%vs 76.47%,P<0.05).Before treatment,there was no significant difference in scores of TCM symptoms and PSQI,total sleep time,sleep latency,awakening time,MT or DA between the two groups(P>0.05).After treatment,scores of TCM symptoms and PSQI,sleep latency and awakening time in combination group were lower than those in control group(P<0.05),while total sleep time and DA were higher than those in control group(P<0.05).The incidence of adverse reactions in combination group was lower than that in control group(7.50%vs 20.93%,P<0.05).Conclusion:The curative effect of Jiugan Kaiyu Decoction combined with zopiclone is significant in elderly patients with chronic insomnia,which can effectively improve sleep quality and regulate neurotransmitters in nervous center.
