润燥止痒胶囊联合他卡西醇软膏对寻常型银屑病患者免疫炎症反应以及肠道微生态的影响

摘　　要：目的:观察润燥止痒胶囊和他卡西醇软膏联合治疗对寻常型银屑病患者免疫炎症反应以及肠道微生态的影响。方法:选取安徽医科大学附属宿州医院2021年3月~2022年12月期间收治的寻常型银屑病患者,共计124例。将其按照随机数字法分为对照组(他卡西醇软膏治疗)和联合组(润燥止痒胶囊联合他卡西醇软膏治疗),各为62例。对比两组疗效、临床症状评分、免疫功能指标、炎症指标及肠道微生态指标。同时记录两组用药安全性。结果:联合组的临床总有效率较对照组更高(P<0.05)。与对照组治疗后相比,联合组大肠埃希菌、肠球菌、银屑病面积与严重指数(PASI)评分、CD8^(+)、白细胞介素-17(IL-17)、肿瘤坏死因子-α(TNF-α)、IL-8更低,皮肤病生活质量指数(DLQI)评分、乳酸杆菌、双歧杆菌、酵母菌、CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)更高(P<0.05)。两组不良反应发生率组间对比未见差异(P>0.05)。结论:寻常型银屑病患者在他卡西醇软膏治疗基础上结合润燥止痒胶囊,可有效改善患者临床症状,调节免疫炎症反应以及肠道微生态,且安全性好。Objective:To observe the effects of combined treatment of runzao zhiyang capsule and tacalcitol ointment on immune inflammatory response and intestinal microecology in patients with psoriasis vulgaris.Methods:Patients with psoriasis vulgaris admitted to Suzhou Hospital Affiliated to Anhui Medical University from March 2021 to December 2022 were selected,a total of 124 cases.Patients were divided into control group(treated with tacalcitol ointment)and combined group(treated with runzao zhiyang capsule combine with tacalcitol ointment)according to the random number method,with 62 cases in each group.The efficacy,clinical symptom scores,immune function indicators,inflammatory indexes and intestinal microecological indexes were compared between two groups.And the safety in two groups was recorded.Results:Compared with control group,the total clinical effective rate in combined group was higher(P<0.05).Compared with control group after treatment,the Escherichia Coli,Enterococcus,psoriasis area and severity index(PASI)score,CD8^(+),interleukin-17(IL-17),tumor necrosis factor-α(TNF-α),and IL-8 in combined group were lower,and the dermatology life quality index(DLQI)score,Lactobacillus,Bifidobacterium,Yeast,CD3^(+),CD4^(+)and CD4^(+)/CD8^(+)were higher(P<0.05).There was no difference in the incidence of adverse reactions between two groups(P>0.05).Conclusion:Patients with psoriasis vulgaris combined with runzao zhiyang capsule on the basis of tacalcitol ointment treatment,which can effectively improve the clinical symptoms of patients,regulate immune inflammatory response and intestinal microecology,and have good safety.
