血液透析患者疲劳及衰弱状况与睡眠的相关性研究

摘　　要：目的:探究并分析血液透析患者的疲劳及衰弱状况与睡眠的相关性情况,为临床开展针对性护理,改善患者睡眠质量并提高其自理能力提供理论依据。方法:采用疲劳量表-14(FS-14)、衰弱筛查量表、自理能力Barthel指数评定量表以及匹兹堡睡眠质量指数量表(pSQI)对本中心接受维持性血液透析患者268例进行调查统计。结果:根据FS-14总得分,本研究患者非疲劳26例,疲劳242例。疲劳患者衰弱状况、睡眠质量、入睡时间、睡眠效率、睡眠障碍以及日间功能障碍得分均高于非疲劳患者,差异均有统计学意义(P<0.05)。Spearman相关分析结果显示,血液透析患者各疲劳因子与衰弱总分、自理能力总分、匹兹堡睡眠质量、入睡时间、睡眠效率、睡眠障碍以及日间功能障碍得分呈正相关(P<0.05)。结论:血液透析患者普遍存在疲劳状况,疲劳状况会降低患者自理能力,加重其衰弱状况,并且会降低其睡眠状况从而导致生存质量下降。Objective:To explore and analyze the correlation between symptoms of fatigue and frailty in hemodialysis patients and their sleep situation,in order to provide theoretical basis for targeted nursing in clinical practice,improve patients'sleep quality,and enhance their self-care ability.Methods:A total of 268 patients undergoing maintenance hemodialysis in our hospital were investigated and statistically analyzed using the fatigue scale-14(FS-14),weakness screening scale,self-care ability Barthel index assessment scale,and Pittsburgh sleep quality index(pSQI).Results:According to the total score of FS-14,there were 26 non-fatigue patients and 242 fatigue patients in this study.Fatigue patients had higher scores in terms of frailty,sleep quality,sleep latency,sleep efficiency,sleep disorders,and daytime dysfunction compared to non-fatigue patients,with statistical significance(P<0.05).The Spearman correlation analysis results showed that each fatigue factor in hemodialysis patients was positively correlated with the total score of weakness,total score of self-care ability,Pittsburgh sleep quality,sleep latency,sleep efficiency,sleep disorders,and daytime dysfunction scores(P<0.05).Conclusion:Hemodialysis patients generally experience fatigue,which can reduce their self-care ability,exacerbate their frailty,and lower their sleep status,leading to a decrease in quality of life.
