墨莲祛白汤联合308 nm准分子激光对肾虚血瘀证白癜风患者白斑面积及血清总补体活性的影响

摘　　要：目的观察墨莲祛白汤联合308 nm准分子激光治疗肾虚血瘀证白癜风的临床疗效。方法本研究采用简单排序随机方案、利用随机数字表法将肾虚血瘀证白癜风患者86例分为对照组及研究组,两组各43例。对照组予308 nm准分子激光,研究组予墨莲祛白汤联合308nm准分子激光。比较两组疗效、白斑面积、白斑消退时间、免疫球蛋白(immunoglobulin,Ig)G、IgM、IgA、血清总补体活性(complement hemolytic activity,CH50)、T淋巴细胞亚群及不良反应。结果研究组总有效率高于对照组(90.70%vs 72.09%,P<0.05)。治疗后,两组的白癜风面积评分指数及IgG、IgM、IgA水平均低于治疗前(P<0.05),且研究组低于对照组(P<0.05)。研究组白斑消退时间早于对照组(P<0.05)。治疗后,两组CH50、CD3^(+)、CD4^(+)及CD4^(+)/CD8^(+)水平高于治疗前(P<0.05),且研究组高于对照组(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论墨莲祛白汤联合308 nm准分子激光能增强肾虚血瘀证白癜风患者免疫功能,缩小白斑面积,加快白斑消退,提高疗效,且未增加不良反应。Objective To observe the clinical efficacy of Molian Qubai decoction combined with 308 nm excimer laser in treating vitiligo of kidney deficiency and blood stasis type.Methods A total of 86 patients with vitiligo of kidney deficiency and blood stasis type were divided into control group and study group by simple ranking random scheme and the random number table method,with 43 cases in each group.The control group received 308 nm excimer laser,and the study group received Molian Qubai decoction combined with 308 nm excimer laser.The efficacy,white spot area,white spot resolution time,immunoglobulin(Ig)G,IgM,IgA,serum complement hemolytic activity(CH50),T lymphocyte subpopulations, and adverse reactions in the two groups were compared. Results The total effective rate of the study groupwas higher than that of the control group(90.70% vs 72.09%, P<0.05). After treatment, the vitiligo area score index,and the levels of IgG, IgM, and IgA in both groups were lower than before treatment(P<0.05), and the study groupwere lower than the control group(P<0.05). The time for white spot regression in the study group was earlier than thatin the control group(P<0.05). After treatment, the levels of CH50, CD3^(+), CD4^(+), CD4^(+)/CD8^(+) in both groups werehigher than before treatment(P<0.05), and the study group was higher than the control group(P<0.05). There was nostatistically significant difference in the incidence of adverse reactions between the two groups(P>0.05). ConclusionThe combination of Molian Qubai decoction and 308 nm excimer laser can enhance the immune function of patients withvitiligo of kidney deficiency and blood stasis type, reduce the area of white spots, accelerate the resolution of white spots,improve the therapeutic effect, and do not increase adverse reactions.
