益元清肺汤联合依替米星治疗气虚肺热证重症肺炎患者的效果及对血清SAA、ChE、CHI3L1的影响

摘　　要：目的 探讨益元清肺汤联合依替米星对气虚肺热证重症肺炎患者的疗效,以及对血清淀粉样蛋白(serum amyloid,SA)A、胆碱酯酶(cholinesterase,ChE)、壳多糖酶3样蛋白(chitinase 3-like protein,CHI3L)1水平的影响。方法 选择我院2019年1月至2022年6月气虚肺热证重症肺炎患者100例,采用抽签法分为对照组及观察组,每组50例。对照组接受依替米星治疗,观察组在对照组治疗基础上加用益元清肺汤,疗程均为2周。比较两组疗效、临床症状、中医证候积分、动脉血气、肺功能、SAA、ChE及CHI3L1,记录不良反应发生情况。结果 治疗后,观察组临床疗效高于对照组,且临床症状改善时间短于对照组(P<0.05)。观察组肺功能指标、ChE水平均高于对照组(P<0.05),中医证候积分、SAA、CHI3L1水平均低于对照组(P<0.05)。两组均未发生严重不良反应(P>0.05)。结论 益元清肺汤联合依替米星治疗气虚肺热证重症肺炎效果显著,可有效改善肺功能和血清SAA、ChE以及CHI3L1水平,值得临床推广。Objective To investigate the effect of Yiyuan Qingfei decoction combined with etemicin on patients with severe pneumonia of Qi deficiency lung heat syndrome,and its effects on serum amyloid(SA)A,cholinesterase(ChE),chitinase 3-like protein(CHI3L)1 levels.Methods A total of 100 cases of severe pneumonia of Qi deficiency lung heat syndrome in our hospital from January 2019 to June 2022 were selected and divided into control group and observation group by drawing lots method,with 50 cases in each group.The control group was treated with etimicin,and the observation group was treated with Yiyuan Qingfei decoction on the basis of the control group.The course of treatment was 2 weeks.The efficacy,clinical symptoms,traditional Chinese medicine(TCM)syndrome scores,arterial blood gas,lung function,SAA,ChE and CHI3L1 were compared between the two groups,and the occurrence of adverse reactions during treatment was recorded.Results After treatment,the clinical efficacy of observation group was higher than that of control group,and the improvement time of clinical symptoms was shorter than that of control group(P<0.05).The levels of lung function index and ChE in observation group were higher than those in control group(P<0.05),and the levels of TCM syndrome score,SAA and CHI3L1 were lower than those in control group(P<0.05).No serious adverse reactions occurred in both groups(P>0.05). Conclusion Yiyuan Qingfei decoction combined with etemicin can effectively improvelung function and serum levels of SAA, ChE and CHI3L1 in the treatment of severe pneumonia with Qi deficiency lungheat syndrome, which is worthy of clinical promotion.
