两种入路结合2.0微型接骨板治疗肩胛骨体部骨折对比研究

摘　　要：目的:探讨2.0mm微型接骨板在双切口入路与Judet入路治疗肩胛骨体部骨折的疗效。方法:回顾性分析2016年7月—2022年7月我院诊治的54例肩胛骨体部骨折患者临床资料,其中27例双切口入路使用2.0微型接骨板治疗的为观察组,27例Judet入路使用2.0mm微型接骨板治疗的为对照组。结果:54例均获得随访,随访率100%,观察组随访12~36个月,平均随访(20.52±6.27)个月,对照组随访14~30个月,平均随访(22.70±5.65)个月。观察组在手术切口长度、术中估计出血量、手术时间、术后住院天数、骨折愈合时间及手术切口并发症上均优于对照组,差异有统计学意义(P<0.05)。重复测量方差分析显示,术后1个月观察组Constant-Murley评分比对照组高7.5(95%CI:5.48~9.55)分,差值具有统计学意义(P<0.001);术后3个月观察组Constant-Murley评分比对照组高7.5(95%CI:4.19~10.74)分,差值具有统计学意义(P<0.001);术后6个月观察组Constant-Murley评分比对照组高9.4(95%CI:6.67~12.19)分,差值具有统计学意义(P<0.05);术后1年Constant-Murley评分比较差异无统计学意义(P=0.261)。结论:2.0微型接骨板用于肩胛骨骨折中疗效良好,双切口入路使用2.0微型接骨板切口短,术中出血少,手术时间短,术后住院天数短,骨折更易愈合,有较好的早期功能,值得临床借鉴。Objective:To explore the comparison of the efficacy of 2.0mm miniature locking plate in double incision approach and Judet approach for the treatment of scaphoid body fracture.Methods:A retrospective analysis was conducted on 54 cases of scaphoid body fractures diagnosed and treated at our hospital,from July 2016 to July 2022.Patients were divided into two groups:the observation group,consisting of 27 cases treated with a double incision technique utilizing a 2.0 miniature bone grafting plate,and the control group,comprising 27 cases treated with a 2.0mm miniature bone grafting plate via the Judet approach.Results:All 54 cases were followed up,achieving a 100%follow-up rate.The follow-up duration ranged from 12 to 36 months,with a mean of(20.52±6.27)months in the observation group and from 14 to 30 months,with a mean of(22.70±5.65)months in the control group.The observation group demonstrated significant advantages over the control group in terms of surgical incision length,estimated intraoperative blood loss,operation duration,postoperative hospital stay,fracture healing time,and complications related to the surgical incision,with all differences being statistically significant(P<0.05).Using repeated-measures ANOVA,the Constant-Murley score in the observation group was found to be 7.5 points higher(95%CI:5.48~9.55)than the control group at 1 month postoperatively,a difference that was statistically significant(P<0.001).At 3 months postoperatively,the Constant-Murley score remained higher by 7.5 points(95%CI:4.19~10.74),with the difference still statistically significant(P<0.001).At 6 months postoperatively,the observation group surpassed the control group by 9.4 points(95%CI:6.67~12.19),with the difference remaining statistically significant(P<0.05).However,at 1 year postoperatively,the Constant-Murley score difference between the observation group and the control group was not statistically significant(P=0.261).Conclusion:2.0 miniature bone grafting plate is used in scaphoid fracture with good efficacy.The us
