摘 要:目的:探讨胎盘组织中可溶性巨噬细胞集落刺激因子受体样酪氨酸激酶1(sFlt-1)、胎盘生长因子(PLGF)的表达及其与子痫前期发生情况的相关性。方法:本研究为随机对照研究,选择2021年6月至2023年6月于榆林市第一医院妇产科分娩的70例子痫前期产妇纳入子痫前期组,年龄(28.42±4.12)岁,年龄范围为22~45岁;另选取同期于榆林市第一医院妇产科分娩的70名血压正常的健康产妇纳入健康组,年龄(27.91±3.86)岁,年龄范围为21~43岁。检测两组产妇胎盘组织中sFlt-1、PLGF的表达水平及胎盘微血管密度(MVD),分析产妇胎盘组织中sFlt-1、PLGF的表达水平与MVD的相关性及对子痫前期的诊断效能。结果:子痫前期组产妇胎盘组织中sFlt-1的表达水平[(1.27±0.36)pg/L]高于健康组[(0.38±0.12)pg/L],PLGF的表达水平[(0.69±0.20)pg/L]低于健康组[(1.14±0.28)pg/L];重度子痫前期产妇胎盘组织中sFlt-1的表达水平[(1.45±0.41)pg/L]高于轻度子痫前期产妇[(1.01±0.29)pg/L],PLGF的表达水平[(0.52±0.13)pg/L]低于轻度子痫前期产妇[(0.86±0.24)pg/L];子痫前期组胎盘组织中的MVD[(101.42±8.95)条/高倍视野]少于健康组[(138.52±14.07)条/高倍视野],差异有统计学意义( P<0.05)。子痫前期组与健康组产妇胎盘组织中MVD与sFlt-1的表达水平( r=-0.412、 r=-0.389)呈负相关( P<0.05),与PLGF的表达水平( r=0.451、 r=0.408)呈正相关( P<0.05)。胎盘组织中sFlt-1联合PLGF诊断子痫前期的灵敏度为90.68%、特异度为61.75%、曲线下面积为0.931。 结论:子痫前期产妇胎盘组织中sFlt-1高表达与PLGF低表达,可能导致胎盘血管生成不足而参与子痫前期的发生。ObjectiveTo investigate the expression of soluble macrophage colony-stimulating factor receptor like tyrosine kinase 1(sFlt-1)and plasma placental growth factor(PLGF)in placental tissues and their correlation with the occurrence of preeclampsia.MethodsThis study was a randomized controlled study,a total of 70 cases of preeclamptic puerpera who received in the department of Obstetrics and Gynecology of the First Hospital of Yulin from June 2021 to June 2023 were selected for inclusion in the preeclamptic group,aged(28.42±4.12)years old,ranging from 22 to 45 years old,and another 70 cases of healthy puerpera with normal blood pressure who delivered in the department of Obstetrics and Gynecology of the First Hospital of Yulin during the same period of time were selected for inclusion in the healthy group,aged(27.91±3.86)years old,ranging from 21 to 43 years old.The expression levels of sFlt-1 and PLGF and placental micro vessel density(MVD)were measured in the placental tissues of the two groups,and the correlation between the expression levels of sFlt-1 and PLGF in the placental tissues of the two groups and MVD was analyzed,as well as the diagnostic efficacy of preeclampsia.ResultsIn the preeclamptic group,the expression level of sFlt-1 in placental tissues of puerpera with severe preeclampsia[(1.27±0.36)pg/L]was higher than that in the healthy group[(0.38±0.12)pg/L],and the expression level of PLGF[(0.69±0.20)pg/L]was lower than that in the healthy group[(1.14±0.28)pg/L];the expression level of sFlt-1 in placental tissues of puerpera with severe preeclampsia[(1.45±0.41)pg/L]was higher than that of puerpera with mild preeclampsia[(1.01±0.29)pg/L];the expression level of sFlt-1 in placental tissues of puerpera with severe preeclampsia[(1.45±0.41)pg/L]was higher than that of puerpera with mild preeclampsia[(1.01±0.29)pg/L],and PLGF expression level[(0.52±0.13)pg/L]was lower than that in mild preeclamptic mothers[(0.86±0.24)pg/L];MVD in the placental tissues of preeclamptic group[(101.42±8.95)bars/high p
