改良注水法在女性患者无痛结肠镜检查中的应用研究

摘　　要：目的探讨改良注水法在女性患者无痛结肠镜检查中的应用价值。方法选择2022年11月—2024年4月因腹部症状就诊于烟台业达医院拟行单人无痛结肠镜检查的女性患者200例作为研究对象,以随机数表法将其分为注气组和改良注水组,每组100例,并记录年龄、身体质量指数(body mass index,BMI)、波士顿肠道准备评分(Boston Bowel Preparation Scale,BBPS)、既往盆腹腔手术史、便秘情况、达盲时间、退镜时间、息肉检出情况、肠道黏膜擦伤情况、结肠镜检查后腹部不适情况,比较两组息肉检出率、肠道黏膜擦伤情况、检查后腹部不适情况。结果改良注水组在无痛结肠镜检查中达盲时间明显低于注气组,差异有统计学意义(P<0.05),两组发现息肉情况、肠道黏粘膜擦伤情况、检查后腹部不适情况方面比较,差异无统计学意义(P>0.05)。结论将改良注水法应用于女性患者无痛结肠镜检查,能够缩短进镜时间,降低结肠镜检查难度,减少麻醉风险,且操作简便易行,值得临床应用。Objective To investigate the application value of modified water infusion in painless colonoscopy for female patients.Methods Random number table method had been used and patients admitted to Yantai Yeda Hospital from November 2022 to April 2024 were divided into air infusion group(100 cases)and modified water infusion group(100 cases).The patient’s age,body mass index(BMI),Boston Bowel Preparation Scale(BBPS),history of abdominal and pelvic surgery,constipation,time of reaching the caecum,reach back to the anus,polyp detection,situation of the intestinal mucosa scratches and abdominal discomfort after colonoscopy were recorded.The time of reaching the caecum was compared between the two groups.Polyp detection,situation of the intestinal mucosa scratches and abdominal discomfort were compared between the two groups.Results The time of reaching the caecum in modifi ed water infusion group was signifi cantly shorter than that in the air infusion group(Z=-3.868,P<0.001),but there was no statistical diff erence in polyp detection,situation of the intestinal mucosa scratches and abdominal discomfort.Conclusion In the application of painless colonoscopy in female patients,the modifi ed water infusion can signifi cantly shorten the time of reaching the caecum,reduce the diffi culty of colonoscopy,reduce the risk of anesthesia,and is easy to operate and implement,with potential for application.
