摘 要:目的:探讨无创高频通气用于治疗新生儿呼吸窘迫综合征的效果。方法:将本院2020年10月至2023年10月收治的106例呼吸窘迫综合征患儿按照治疗方法进行分组,其中对照组患儿采用经鼻持续气道正压通气治疗(n=53),研究组患儿接受无创高频振荡通气治疗(n=53)。将两组患儿治疗前、治疗后72 h血气分析指标,肺功能指标,呼吸暂停发生情况与无创通气失败率,以及并发症发生情况进行对比。结果:治疗后72 h两组患儿氧合指数(Oxygenation index,OI)、动脉血氧分压(Pulmonary arterial oxygen tension,PaO_(2))及研究组pH值均较治疗前升高,且研究组比对照组高,而两组患儿动脉血二氧化碳分压(Arterial carbon dioxide partial pressure,PaCO_(2))均较治疗前下降,且研究组比对照组低;治疗后72 h两组患儿吸气时间(Inhalation time,Ti)、呼气时间(Expiratory time,Te)及潮气量(Tidal Volume,VT)相较于治疗前均上升,且研究组比对照组高;研究组患儿呼吸暂停发生率、无创通气失败率均比对照组低,呼吸暂停消失时间相较于对照组缩短(P<0.05)。结论:无创高频通气可促进患儿血气分析指标的改善,同时改善其肺功能指标。Objective:To investigate the effect of non-invasive high-frequency ventilation in the treatment of neonatal respiratory distress syndrome.Methods:A total of 106 children with respiratory distress syndrome admitted to our hospital from October 2020 to October 2023 were divided into two groups according to treatment methods.The control group received nasal continuous positive airway pressure(n=53),and the study group received non-invasive high-frequency oscillatory ventilation(n=53).Blood gas analysis indexes,pulmonary function indexes,apnea occurrence,non-invasive ventilation failure rate,and complications were compared between the two groups before and 72 hours after treatment.Results:After treatment,the oxygenation index(OI),arterial oxygen pressure(PaO_(2)),and pH value of the study group in both groups significantly increased compared to pretreatment(P<0.05),and the study group was higher than the control group(P<0.05).However,the arterial carbon dioxide pressure(PaCO_(2))of both groups decreased compared to pretreatment,and the study group was lower than the control group(P<0.05).After treatment,the inhalation time(Ti),expiratory time(Te),and tidal volume(VT)of both groups obviously increased compared to pretreatment(P<0.05),and the study group was higher than the control group(P<0.05).The incidence of respiratory pauses and the failure rate of non-invasive ventilation in the study group were lower than those in the control group,and the disappearance time of respiratory pauses was shortened compared to the control group(P<0.05).Conclusion:Non-invasive high frequency ventilation can promote the improvement of blood gas analysis index and lung function index of children.
