三维超声心动图联合M型超声对心肌梗死后室壁运动异常的评估分析

摘　　要：目的:探讨三维超声心动图(Three-Dimensional Echocardiography,3DE)联合M型超声对心肌梗死(Myocardial infarction,MI)后室壁运动异常(Regional wall motion abnormality,RWMA)的评估价值。方法:选取2022年10月至2023年10月期间本院收治的77例MI患者作为研究对象。所有患者均行3DE、M型超声及冠状动脉造影(Coronary angiography,CAG)检查RWMA发生情况。以CAG检查结果为标准,比较3DE、M型超声、3DE联合M型超声评估MI后RWMA与CAG结果的价值和一致性。结果:CAG检查结果显示,77例MI患者中46例为RWMA,31例为非RWMA。3DE联合M型超声评估MI后RWMA的灵敏度、准确度、阴性预测值高于单独3DE、M型超声检查(P<0.05)。3DE或M型超声评估MI后RWMA结果与CAG结果的一致性均较强(P<0.05)。3DE联合M型超声评估MI后RWMA结果与CAG结果的一致性强(P<0.05)。结论:较单独3DE或M型超声检查,3DE联合M型超声评估MI后RWMA的价值较高。Objective:To evaluate the value of three-dimensional echocardiography(3DE)combined with M-mode ultrasound in the evaluation of ventricular wall motion abnormalities(RWMA)after myocardial infarction(MI).Methods:77 patients with MI admitted to our hospital from October 2022 to October 2023 were selected as the study objects.All patients underwent 3DE,M-mode ultrasound and coronary angiography(CAG)to examine the occurrence of RWMA.Using the results of CAG as the standard,the value and consistency with CAG results were compared with 3DE,M-mode ultrasound and 3DE combined M-mode ultrasound.Results:CAG results showed that 46 of 77 patients with MI had RWMA and 31 had non-RWMA.The sensitivity,accuracy and negative predictive value of RWMA after 3DE combined with M-mode ultrasonography were higher than those of 3DE and M-mode ultrasonography alone(P<0.05).There was a strong consistency between the CAG results and 3DE,or M-mode ultrasound,there was a stronger consistency between CAG results and 3DE combined M-mode ultrasound(P<0.05).Conclusion:Compared with 3DE or M-mode ultrasonography alone,3DE combined with M-mode ultrasonography is more valuable in assessing RWMA after MI.
