摘 要:目的:探讨麻桂柴苓汤治疗慢阻肺急性加重期(Acute chronic obstructive pulmonary disease,AECOPD)的疗效。方法:选取2022年7月至2023年12月期间本院收治的97例AECOPD患者作为研究对象,随机数表法将患者分为对照组(n=48)和研究组(n=49)。对照组患者接受常规西医对症治疗;研究组在对照组基础上增加麻桂柴苓汤治疗。分析比较两组的中医证候评分、血清炎症因子水平、血气指标、肺功能指标及不良反应发生率。结果:治疗4 w后,两组的中医证候评分、白介素-6(Interleukin-6,IL-6)、C反应蛋白(C-reactive protein,CRP)、肿瘤坏死因子-ɑ(Tumor necrosis factor-ɑ,TNF-ɑ)水平、二氧化碳分压(Partial pressure of carbon dioxide,PaCO_(2))均明显降低,且研究组均明显低于对照组(P<0.05);两组的动脉血氧分压(Partial pressure of arterial oxygen,PaO_(2))、血氧饱和度(Oxygen saturation,SaO_(2))、第1秒用力呼气容积(Forced expiratory volume in the first second,FEV1)、用力肺活量(Forced vital capacity,FVC)、FEV1/FVC均明显升高,且研究组的升高幅度明显高于对照组(P<0.05)。治疗期间,两组均未发生不良反应。结论:麻桂柴苓汤治疗可缓解AECOPD患者症状,改善体征,减轻炎症反应,改善患者肺功能和血气指标,且不增加不良反应。Objective:To investigate the curative effect of Magui Cheling Decoction in the treatment of acute COPD exacerbation(AECOPD).Methods:Ninety-seven patients with AECOPD admitted to our hospital from July 2022 to December 2023 were selected as the study objects.Patients were divided into control group(n=48)and study group(n=49)by random number table method.Control group patients received conventional Western medicine symptomatic treatment;The study group was treated with Magui Chaling decoction on the basis of control group.TCM syndrome score,serum inflammatory factor level,blood gas index,lung function index and incidence of adverse reactions were analyzed and compared between the two groups.Result:After 4 weeks of treatment,the scores of TCM syndromes,interleukin-6(IL-6),C-reactive protein(CRP),tumor necrosis factor-ɑ(TNF-ɑ)and partial pressure of carbon dioxide(PaCO_(2))in the two groups were significantly decreased,and those in the study group were significantly lower than the control group(P<0.05).The partial pressure of arterial oxygen(PaO_(2)),Oxygen saturation(SaO_(2)),forced expiratory volume in the first second(FEV1),Forced vital capacity(FVC)and FEV1/FVC were significantly increased in both groups,and the increases in the study group were significantly higher than those in the control group(P<0.05).During treatment,no adverse reactions occurred in both groups.Conclusion:Magui Chaling Decoction can relieve symptoms,improve signs,reduce inflammation,improve lung function and blood gas indexes in patients with AECOPD,and does not increase adverse reactions.
