摘　　要：目的探讨采用输卵管结扎术和输卵管切除术对单侧输卵管积水患者卵巢储备功能及血流参数的影响。方法选择2020年1月~2023年6月浙江省嘉兴市妇幼保健院因单侧输卵管积水行手术治疗的患者共93例,随机分为切除组(45例)及结扎组(48例),比较两组术前及术后6个月手术指标(手术时间,术中出血量)、卵巢储备功能[血清卵泡刺激素(FSH),黄体生成素(LH),雌二醇(E_(2)),抗缪勒氏管激素(AMH)]、卵巢血流参数[卵巢血管指数(VI),血流指数(FI),血管血流指数(VFI)]的变化。结果(1)两组手术时间和术中出血量差异无统计学意义(P>0.05)。(2)术前,两组血清AMH、FSH、LH及E_(2)水平差异无统计学意义(P>0.05);术后6个月,两组血清AMHFSH、LH水平与术前比较差异无统计学意义(P>0.05),两组间血清AMH、FSH、LH及E_(2)水平差异无统计学意义(P>0.05)。(3)①单侧输卵管切除术中,与术前比较,术后6个月VFI明显降低(P<0.01),健侧差异无统计学意义(P>0.05);健侧及患侧VI及FI差异均无统计学意义(P>0.05);②单侧输卵管结扎术中,术后6个月与术前比较,患侧及健侧VI、FI、VFI差异均无统计学意义(P>0.05)。结论术后6个月内,单侧输卵管切除术及结扎术对卵巢储备功能影响不明显,但单侧输卵管切除术可能会降低患侧卵巢血流灌注。Objective To investigate the effects of tubal ligation or salpingectomy on ovarian reserve function and blood flow parameters in patients with unilateral hydrosalpinx.Method A total of 93 patients who underwent surgical treatment for unilateral hydrosalpinx at Jiaxing Maternal and Child Health Hospital in Zhejiang Province from January 2020 to June 2023 were randomly divided into a resection group(45 cases)and a ligation group(48 cases).The changes in surgical indicators(surgery time,intraoperative blood loss),ovarian reserve function[serum fllicle stimulating hormone(FSH),luteinizing hormone(LH),estradiol(E_(2)),anti Mullerian hormone(AMH)],and ovarian blood flow parameters[ovarian vascular index(VI),blood flow index(FI),vascular blood flow index(VFI)]were compared between the two groups before and 6 months after surgery.Results(1)There was no statistically significant difference in surgical time and intraoperative bleeding between the two groups(P>0.05).(2)Before surgery,there was no statistically significant difference in the levels of serum AMH,FSH,LH,and E_(2) between the two groups(P>0.05);Six months after surgery,there was no statistically significant difference in the levels of serum AMH,FSH,and LH between the two groups compared to preoperative levels(P>0.05),and there was no statistically significant difference in the levels of the above four indicators between the two groups(P>0.05).(3)During unilateral salpingectomy,compared with preoperative results,the unilateral VFI significantly decreased at 6 months postoperatively(P<0.01),while there was no statistically significant difference on the healthy side(P>0.05);There was no statistically significant difference in VI and FI between the healthy and affected sides(P>0.05)During unilateral salpingectomy,there was no statistically significant difference in VI,FI,and VFI between the affected and healthy sides 6 months after surgery compared to preoperative levels(P>0.05).Conclusion Within 6 months after surgery,unilateral salpingectomy and ligation have no s
