曹继刚从“肝虚肾实”论治勃起功能障碍

摘　　要：曹继刚教授认为“肝虚”“肾实”为勃起功能障碍的重要病机。“肝虚”即肝气血阴阳亏虚,致肝藏血和疏泄功能失司;“肾实”即风寒湿热瘀等实邪阻滞肾中,致肾藏精和主水功能下降。治疗上,“肝虚”治以补,顺气通达,肝阴虚以一贯煎加减,可搭配丹栀逍遥散,肝血虚以当归芍药散加减,肝气虚选补中益气汤合曲直汤,肝阳虚方选暖肝煎加减;“肾实”治以泻,疏泻病邪,肾风证以升麻胃风汤加温肾引经药治疗,肾湿证用十枣汤合龟鹿二仙膏,肾实寒选四逆散合济川煎,肾实热以舟车丸合生脉散加减,肾血瘀证处以四物汤合补阳还五汤。Professor CAO Jigang believes that“liver deficiency”and“kidney excess”are the important pathogenesis of erectile dysfunction.“Liver deficiency”is the deficiency of liver qi,blood and yin-yang,which leads to the loss of liver blood storage and drainage function.“Kidney excess”means that wind-cold,dampness-heat,stasis and other sthenia pathogenic factors block the kidney,causing the kidney’s function in storing essence and regulating water decline.In terms of treatment,the treatment for“liver deficiency”is to tonify and promote qi circulation,for liver yin deficiency,modified Yiguan Decoction(一贯煎)combined with Danzhi Xiaoyao Powder(丹栀逍遥散)can be used;for liver blood deficiency,modified Danggui Shaoyao Powder(当归芍药散)can be used;for liver qi deficiency,Buzhong Yiqi Decoction(补中益气汤)combined with Quzhi Decoction(曲直汤)can be used;for liver yang deficiency,modified Nuangan Decoction(暖肝煎)can be used.The treatment for“kidney excess”is to purge,purge the pathogenic factors,Shengma Weifeng Decoction(升麻胃风汤)combined with medicinal for warming the kidney to guide meridians is used to treat the syndrome of kidney-wind;Shizao Decoction(十枣汤)combined with Guilu Erxian Paste(龟鹿二仙膏)is used to treat kidney dampness syndrome;Sini Powder(四逆散)combined with Jichuan Decoction(济川煎)is used to treat kidney excess cold syndrome;modified Zhouche Pill(舟车丸)combined with Shengmai Powder(生脉散)is used to treat kidney excess heat syndrome;and Siwu Decoction(四物汤)combined with Buyang Huanwu Decoction(补阳还五汤)is used to treat kidney blood stasis syndrome.
