基于扶阳思想探讨卒中后抑郁病机及治法

摘　　要：近年来,卒中后抑郁的发病率呈逐年上升趋势,已成为严重的社会和公共卫生问题。扶阳学派从《黄帝内经》《伤寒论》思想出发,在辨证方面注重“阴阳辨证”,以“阳主阴从观”为理论基础,认为中风后“阳气运行失常”是卒中后抑郁的核心病机,并以“通阳”和“温阳”为治疗总则,提出并贯彻“三焦次第疗法”,在治疗上倡导先宣通中上二焦,再温补下焦元阳,使阴阳回归本位,五脏元真通畅则病愈,于临证实践中收效显著,为卒中后抑郁的辨证治疗提供了新思路。In recent years,the incidence of post⁃stroke depression(PSD)has been increasing year by year,which has become a serious social and public health problem.Starting from the emphasis on yang qi in the Yellow Emperor’s Internal Classic and Treatise on Febrile Diseases,Fuyang school believes that the dysfunction of yang qi after stroke is the core pathogenesis of PSD,based on the theory of“yang governs yin”.It also proposes and implements the“sanjiao regular sequence therapy”with the general principles of“clearing yang”and“warming yang”.In terms of treatment,it is advocated to first publicize the middle and upper jiao,and then warm up the lower one.So as to return yin and yang to their original state,then the five viscera are truly unobstructed,the disease will recover,and the effect is significant in clinical practice,providing a new approach for the syndrome differentiation treatment of PSD.
